About this file
This document is an Attachment D.6 Deliverables/Reports for a VA Healthcare Services contract for a Community-Based Outpatient Clinic (CBOC), specifically for the Dyersburg and Holly Springs locations. The comprehensive table details numerous reporting and submission requirements for the contractor, including mandatory deliverables to be submitted with the proposal and throughout the contract lifecycle.
Key submission requirements include past performance references, facility information, management approach details, staff certifications and licenses, subcontracting plans, pricing schedules, and various ongoing reports such as patient complaint summaries, adverse event reports, time and attendance logs, and workload reports. Submissions are to be made to the Contracting Officer/Contract Specialist (CO/CS) or Contracting Officer Representative (COR), with compliance linked to specific sections of the Performance Work Statement (PWS) and technical evaluation factors. Critical ongoing requirements include background investigations, credentialing, cyber security training, Joint Commission accreditation, and monthly/quarterly reporting on items like AED device checks and The Joint Commission (TJC) standards audits.
