About this file

This document is an Amendment (Amendment 0005) to a Department of Veterans Affairs (VA) solicitation for Community Based Outpatient Clinic (CBOC) Services in Dyersburg, Tennessee. The amendment extends the proposal submission deadline to January 27, 2026, at 10:00 AM Central Time and provides answers to vendor questions regarding space requirements, hallway dimensions, and enrollment projections. Key modifications include updating wage determinations (adding Attachment D.20 Davis-Bacon Act Wage Determination and replacing Attachment D.9 with a revised Service Contract Act wage determination), and revising the price schedule to reflect a new period of performance from October 1, 2026, through September 30, 2036.

The solicitation covers primary and mental health care services with a base period and nine option periods, each 12 months long. The contract includes per-member-per-month (PMPM) pricing for primary health care and mental health services, with patient enrollment ranging from 1,911 in the base year to 1,985 in the final option year. Additionally, the amendment addresses vendor inquiries about facility requirements, such as reducing the minimum square footage from 16,500 to 10,000 square feet, and allowing flexibility in room and hallway configurations while maintaining service delivery capabilities. Proposals and past performance surveys must be emailed to laura.edelman@va.gov for consideration.

Vendor Questions
VA Answers
100
Section 8 SPACE requirements states “There will be one (1) teamwork room that combined will accommodate a total of twenty (20) personnel.”

Question - Would the VA allow the teamwork to appropriately sized for the contractors staffing plan?

Space should accommodate at least 20 people.

101
Will the VA accept 5 foot wide hallways which meet ADA requirements?
Hallways have to be 6 foot wide.
102
The most recent PC enrollment is 1716, but the Base CLIN is 1911. That is an 11% increase in current PC enrollment. Would the government consider adjusting the CLIN closer to the most recent enrollment on the schedule of services?
Enrollment numbers can fluctuate. The Base LIN is the best estimate that the Medical Center can provide based on previous years and may include projected enrollment increases.
103
The VA has asked for a 16,500 square foot minimum for the new clinic. Will the VA waive the 16,500 square foot minimum requirement if the offeror meets all room and hallway requirements? We believe the 16,500 square foot requirement is a significant and unnecessary cost driver. And the VA's answer: There has to be enough space for the service requested. Does the existing CBOC have enough space for the service that's requested?
Minimum square footage is adjusted to 10,000 sq, the need was estimated to be 16,500. The Medical Center wants to see what contractors can propose for the most sufficient space design.

The following price schedule replaces the schedule in the original RFP; this is to provide an accurate period of performance.
