About this file
This document is an Attachment D.6 Deliverables/Reports table for a VA Healthcare Contract (Solicitation 36C24925R0056) for the Dyersburg Community-Based Outpatient Clinic (CBOC). The comprehensive deliverables matrix outlines numerous required submissions from the contractor to the Contracting Officer/Contract Specialist (CO/CS) and Contracting Officer Representative (COR), organized by submission timing and compliance requirements.
Key submission requirements include providing past performance references, facility information, management approach details, continuity of care plans, staff certifications and licenses, subcontracting plans, pricing schedules, and various ongoing reports such as patient complaint summaries, malpractice claims, workload reports, and quarterly Joint Commission standards audits. Critical submission points occur with the initial proposal, upon contract award, periodically during contract performance, and at annual contract renewals. The document emphasizes contractor responsibility for accuracy, completeness, and timely submission of all required documentation, with specific compliance references to the Performance Work Statement (PWS) sections and federal acquisition regulations.
