About this file

This document is a procedural guide for the REACH VET (Recovery Engagement And Coordination for Health - Veterans Enhanced Treatment) program, detailing six key steps for healthcare providers serving high-risk veterans. The steps include receiving a notification about a high-risk veteran, optionally accessing a dashboard, re-evaluating the veteran's care comprehensively, considering treatment enhancement strategies, conducting outreach using specific talking points, and documenting interactions in the CPRS (Computerized Patient Record System).

Providers are expected to complete all tasks by the fourth Wednesday of the month and reach out to identified veterans within one week of notification. The detailed process involves reviewing electronic health records, screening evaluations, diagnoses, and treatment plans, ensuring access to needed services, and implementing strategies such as enhanced communication, safety planning, increased monitoring of stressful life events, and interventions to improve coping strategies. The goal is to provide proactive, collaborative care that addresses the veteran's specific clinical needs and potential risks.
