About this file
This document is an Attachment D.6 Deliverables/Reports for a VA Healthcare Services contract, specifically for the Dyersburg and Holly Springs Community-Based Outpatient Clinics (CBOC). The comprehensive table outlines numerous reporting and submission requirements for the contractor, including pre-award and post-award deliverables such as past performance references, facility information, management plans, staffing details, certifications, licensing documentation, and ongoing operational reports. Key submission requirements include providing staff names and roles, subcontracting plans, pricing schedules, laboratory accreditation certificates, background investigations, cyber security training certificates, patient safety reports, time and attendance logs, and quarterly Joint Commission standards audits.
Contractors must submit various documents to the Contracting Officer/Contract Specialist (CO/CS) and Contracting Officer Representative (COR), with most submissions required with the initial proposal or upon contract award. The documents are tied to specific Technical Evaluation Factors and Performance Work Statement (PWS) sections, covering areas like healthcare operations experience, facility management, continuity of care, staff credentialing, patient safety, and compliance. Notable requirements include obtaining PIV cards for personnel, maintaining active staff credentialing, submitting monthly AED device checks, establishing a Women Veteran Champion plan, and providing annual subcontracting performance reports. The solicitation also notes that offerors must provide key personnel resumes with details about qualifications, licenses, experience, education, and certifications.
