About this file
This is a Past Performance Survey form used by the Federal Aviation Administration (FAA) to evaluate contractor performance on federal contracts relevant to solicitation 6973GH-25-R-00221 for Aviation Safety (AVS) Safety, Analytical and Technical Support Services (SATSS) II.
The survey is completed by government points of contact who worked with contractors on previous similar efforts. Respondents provide contract details including contractor name, contract number, award date, period of performance, total contract amount, and contractor role (prime or subcontractor). The form documents any deficiency notices, cure notices, or show cause orders issued during performance. Evaluation criteria are organized into five performance categories: Cost Control (contractor's adherence to agreed-to price and use of innovation/efficiencies); Quality and Timeliness of Services (schedule performance, quality control plans, responsiveness, data deliveries, and invoice accuracy); Business Relations and Effective Management (organizational structure, subcontractor teaming, workforce retention, corporate office responsiveness, employee utilization, emergency response, and compliance with contract terms); Customer Satisfaction (overall satisfaction and customer support effectiveness); and Integrity (problem identification and corrective action, ethical conduct, transparency regarding employee issues, and history of cure/show cause notices). Each criterion uses a standardized five-point rating scale: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), or Not Applicable (N/A). Completed surveys are due to Contracting Officer Cynthia Cooper at cynthia.cooper@faa.gov no later than March 24, 2026, at 2:00 p.m. CST.
