About this file
This is a Past Performance Questionnaire (PPQ) template used as an evaluation attachment for the Minot AFB Flooring MACC FY26 solicitation (FA4528-26-R-0001). The template is designed to collect candid assessments from clients or project overseers regarding a contractor's historical performance on relevant projects. Offerors must identify no more than three (3) recent and relevant projects in Section 2 and distribute the questionnaire to the responsible client contact, who completes Section 3 and submits directly to the Air Force Contracting Officer at 5CONS.PKAC.FlooringMACC@us.af.mil prior to the proposal deadline. Questionnaires submitted directly by offerors will not be evaluated.
The performance evaluation uses CPARS-based rating standards spanning five key areas: Quality of Work (contract conformance, workmanship, and quality control effectiveness); Schedule (timeliness, delay mitigation, and administrative document submission); and Management & Business Relations (management responsiveness, subcontractor management, professionalism, and site safety). Respondents rate performance using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) and must provide narrative explanations for any Marginal or Unsatisfactory ratings. The template concludes with an overall assessment question asking whether the respondent would award another contract to the contractor.
