About this file

This is a Past Performance Questionnaire used to evaluate contractor performance on the PM MAV Logistic Fielding Support Services contract opportunity. The questionnaire is to be completed by government points of contact who have direct knowledge of a contractor's prior performance and submitted to darlamae.d.lasky.civ@army.mil and jill.t.wagner.civ@army.mil.

The questionnaire requires evaluation across ten performance areas using a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) plus "Too Soon to Tell" and "Not Applicable" options. Evaluated areas include: effectiveness of program management and subcontractor control; adherence to contracted costs; parts forecasting and supply management; timeliness of service and product deliverables; technical support and maintenance through Field Service Representatives; New Equipment Training and Displaced Equipment Training recruitment and delivery; deprocessing activities and welding standards compliance; Modified Work Order installations and configuration management; and quality management standards. For each rating, respondents must provide explanations and assess the extent of contractor fault versus government actions or external factors contributing to any adverse issues. The questionnaire also requests identification of other relevant past efforts and special accomplishments. Completion requires identification information from both the offeror's representative and the government respondent providing the evaluation.
