About this file

This is a Past Performance Questionnaire designed to evaluate contractor performance on relevant past contracts as part of the source selection process for the Whiteman AFB Airfield Pavement IDIQ program. The questionnaire serves as a template for references and past customers to provide detailed assessments of a bidding contractor's capabilities and performance history.

The evaluation focuses on a five-year option contract (base year plus four one-year options) to provide comprehensive maintenance, repair, and minor construction services for runway and airfield pavements at Whiteman Air Force Base in Missouri. Respondents are asked to rate contractor performance across five key categories: Management (12 criteria including staffing, supervision, problem-solving, and payment practices); Schedule (2 criteria addressing schedule development and adherence); Quality (4 criteria covering quality control and deficiency correction); Regulatory Compliance (2 criteria related to federal requirements and permitting); and Overall Performance (assessed on a scale ranging from Exceptional to Unsatisfactory). The questionnaire requests supporting information for each rating and includes yes/no questions regarding cure notices and future contract willingness. Responses should be submitted via email to the 509th Contracting Squadron point of contact, SrA Nathaniel Smith, with contact information provided. Upon completion, the questionnaire becomes part of the official source selection record and is controlled as Source Selection Information.

ATTACHMENT 5 - PAST PERFORMANCE QUESTIONNAIRE

The 509th Contracting Squadron of the Air Force Global Strike Command is in the process of competitively selecting a source for the Whiteman AFB Airfield Pavement IDIQ program. The work accomplished for this requirement consists of providing all labor, supplies, materials, supervision, transportation, equipment and all else necessary to execute the maintenance and repair of Airfield Pavements contract at Whiteman AFB, MO, as defined in the Statement of Work (Base year and 4 one-year options). Projects consist of a broad range of maintenance, repair, and minor construction of runway and airfield pavements. One of the considerations in proposal evaluation is the offerors’ past performance on contracts or other work efforts which reflect the offeror’s ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror’s performance on those contracts or work efforts. Our areas of interest regarding the offeror are summarized in the enclosed questionnaire.

To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to 509cons.sollgca@us.af.mil, caleb.kirchhoff@us.af.mil, james.mccoy.17@us.af.mil, and nathaniel.smith.68@us.af.mil. Please call SrA Nathaniel Smith at 660-687-2592 if you have any questions. Your completed questionnaire will become a part of the official record. Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this evaluation.
