About this file
This document is the State of Illinois Scope of Services Appendices for the Damage Claim Sign, Procedures, Form, and Statistics related to vehicle emissions testing. The appendix outlines the procedures for submitting a damage claim if a vehicle is damaged during or as a result of an emissions test at an emissions testing facility. Claimants must submit a completed damage claim form within 30 days of the test date and any supporting documentation within an additional 30 days. The contractor, [Contractor Name], is responsible for handling and responding to damage claims, with a goal of resolving each claim within 60 days of receiving the form.
The appendix also provides historical damage claim statistics, showing a decrease in claims from 48 in 2017 to 16 in 2021. The document references a related state and local contract opportunity for vehicle emissions testing, but notes that it is not open for vendors to bid as it was previously cancelled due to an IEPA technical error.

Your vehicle’s VIN can be found on the driver's side dashboard in the corner near the windshield, or on the label on the inside of the driver's side door (where the door latches when it is closed).
