About this file
This document is the Appendix 13 - Example Vehicle Inspection Reports and Repair Data Form for an Illinois state vehicle emissions testing contract opportunity. It provides samples of Vehicle Inspection Reports (VIR) for both passing and failing inspections, as well as an Example Repair Data Form to be used when a vehicle fails the OBD emissions test. The VIRs detail the results of the OBD system check, including information on the vehicle's diagnostic trouble codes, system monitors, and malfunction indicator lamp status. The Repair Data Form outlines the required information to be submitted by repair facilities prior to the vehicle being retested, including details on the repairs performed and the emissions-related costs.
The document also includes a list of the closest recognized repair shops to the vehicle inspection facilities, along with information on the number of repairs performed and their success rate. Lastly, it provides guidance to vehicle owners on next steps if their vehicle fails the emissions test, including recommended actions and the importance of having repairs performed by a qualified technician.

Illinois Vehicle Inspection Report - Version 16.1
Congratulations, the vehicle listed below has passed and meets Illinois testing standards. The emission test requirement for renewal of the vehicle's registration has been satisfied and Registration Renewal Authorized. Thanks for helping clean our air! This test was performed in accordance with 35 Ill Admin Code Parts 240 and 276. Your vehicle may be covered under warranty pursuant to Section 207 of the Clean Air Act.
This vehicle received an On-Board Diagnostic (OBD) system check and passed. The OBD test checks for: the illumination of the Malfunction Indicator Lamp/Light (MIL), also known as the Check Engine Light; Diagnostic Trouble Codes (DTCs) stored in the vehicle’s computer system; vehicle specific system monitors; and the status of the data link connector (DLC). At this time, the vehicle passed the Illinois vehicle emissions testing standards. Please keep in mind if the Check Engine Light comes on and stays illuminated before your next test is due, we recommend you have your vehicle looked at by a recognized repair technician, and together we can help clean the air we breathe! However if the light flashes, we strongly encourage you to seek immediate repair.

Your vehicle's computerized self-diagnostic system (OBD) registered the DTC’s listed above. This is probably an indication of a malfunction of an emission component. However, multiple and/or seemingly unrelated faults may be an indication of an emission-related problem that occurred previously, but upon further evaluation by the OBD system was determined to be only temporary. Possible causes of the DTC may vary which may include several components and/or systems. Therefore, proper diagnosis by a qualified technician is required to positively identify the source of any emission-related problem. Please provide the Repair Data Form to the repair shop for submission of required repair data online prior to retesting the vehicle. This will expedite your retest experience. After the necessary repairs have been performed, please return to a test station to have the vehicle retested. See the Repair Data Form for more information.

Repair Data Form V2621457180526
Why did my vehicle not pass?
Specific details for your test are printed on the Vehicle Inspection Report. Your vehicle likely did not pass for one or more of the following reasons:
• Diagnostic trouble codes – from a vehicle component problem that could result in excessive emissions
• The OBD System is inoperative
• The Diagnostic Link Connector (DLC) is damaged, inaccessible or missing
• An insufficient number of system monitors were set to ready; or,
• Specific system monitors were not supported
My vehicle runs fine; so why did it not pass?
Generally, emission control devices have little impact on the drivability of a vehicle. Your vehicle may seem to be operating fine but still has an emission related problem. Even the best maintained vehicles might experience these types of problems.
Can I do my own repairs?
Yes, but today's modern fuel-injected and computer-controlled vehicles present unique challenges. Emissions problems generally require a high degree of expertise and training to diagnose and repair.
What do I do now?
• Get your vehicle repaired and retested as soon as possible. Motorists are free to choose any repair shop.
• We recommend you consider having your vehicle repaired by a recognized repair technician: a person professionally engaged in vehicle repair, employed by a going concern whose purpose is vehicle repair, or possessing nationally recognized certification for emission related diagnosis and repair.
• Visiting a recognized repair technician will help ensure your vehicle is ready to be tested
• Before leaving the repair facility, ask your repair technician to enter in the necessary repair data online or fill out the required repair information below.
Retests are not allowed without proper repair data.
Your vehicle may be covered under warranty pursuant to Section 207 of the Clean Air Act (42 USC 7541). Check with your repair facility, dealership or manufacturer for complete warranty coverage.
Illinois Repair Data Form - Version 18.1
*V2621457180526*
This vehicle was rejected from testing because it did not successfully complete all the required self-tests, therefore the OBD test could not be completed.
This generally results if the battery was recently disconnected or the vehicle was recently repaired, in which case the vehicle must complete one or more manufacturer-recommended drive cycles. Several days of normal driving usually will cause all the monitors to reset to a status of READY. If, however, this is the second or third time that the vehicle is rejected for readiness, we recommend you seek assistance from a recognized repair technician and provide them with the Repair Data Form (RDF) to complete. Recognized repair shops can submit the repair data online which should expedite your retest experience as long as the shop enters the data prior to your retest. In any case, be sure to bring a copy of the completed RDF with you when you take your vehicle to be tested again.
