About this file
This Addendum #4 document modifies the contract specifications for RFP-KTHFS-2026-003, a general construction project by Klamath Tribal Health and Family Services (KTHFS) for a Chiloquin Fitness Center Expansion in Chiloquin, Oregon. The project involves construction of a 20,000+ square foot addition to the existing fitness center, including a community room, workout room, teaching kitchen, wellness center, smoothie bar, offices, restrooms, and showers. The addendum establishes a final question deadline of Thursday, October 1, 2026, at 12:00 p.m., with a submission deadline of Wednesday, October 7, 2026. KTHFS will remove existing furnishings and fitness equipment before work commences and will supply and install all moveable items, furnishings, exercise equipment, kitchen appliances, and restroom accessories, with the contractor responsible for installation of these owner-supplied items. The contractor must maintain all emergency exits, coordinate operations with KTHFS facilities staff to minimize disruption to the occupied adjacent building, limit utility outages to unoccupied hours, and provide seven business days' notice before disrupting life safety systems.
Addendum #4 specifically replaces Section 23 33 00 (Air Duct Accessories) with detailed specifications for sheet metal materials, backdraft dampers, control dampers, access doors, fire dampers, turning vanes, and flexible connectors. Approved manufacturers include Ruskin, Greenheck, Nailor, Prolon, and equivalent products. The addendum clarifies that all HVAC controls are packaged systems, removing references to separate instrumentation specifications; confirms NEMA 3R automatic transfer switches rated at 100kAIC are required; and specifies that data outlet rough-in only is the contractor's responsibility, with cabling and terminations to be performed by KTHFS or their vendors. The addendum also addresses generator sizing at 5,000-foot altitude elevation, confirming no upsizing or derating is required, and recommends Prolon for variable volume terminal (VVT) zone control systems as standard off-the-shelf equipment selected by the mechanical subcontractor.

ADDENDUM NO. 004
TO ALL PLAN HOLDERS:
This Addendum forms a part of the Contract Documents and modifies the original bidding documents released for bid preparation. Please notify everyone concerned as your sub-contractors and suppliers of the issuance and contents of this addendum prior to date of bid opening.
THE FINAL DATE RFP QUESTIONS WILL BE ACCEPTED IS THURSDAY, OCTOBER 1, 2026, AT 12:00 P.M.
QUESTIONS AND CLARIFICATIONS:
Q1: Spec 23 33 00 Air Duct Accessories, 2.08, D references spec section 23 09 00 Instrumentation and
Controls for HVAC as regards actuators for control dampers (BD-1. ZD-1, ZD-2, ZD-3). I am not finding that spec section in the book. Can this be provided?
A1: All controls are packaged on this project so spec section 23 09 00 Instrumentation and Controls for HVAC is irrelevant and the reference has been removed.
Q2: I noticed the altitude is up to 5000’, was this altitude taken into consideration when sizing the generators? Or are you looking for us to upsize and derate our gens based on that elevation and kW provided in the spec?
A2: No need to upsize and derate generators.
Q3: The one-line has the 2000A ATS as NEMA 3 but the spec indicates NEMA 1. Could you please confirm which is needed for the application?
A3: NEMA 3R is required.
Q4: 100KAIC is specifically called out for the 2000A ATS. Is this accurate or is 65kAIC acceptable for the
2000A ATS?
A4: No, ATS must be 100kAIC.
Q5: At both projects there is an RTU which uses a zoning system, but no control protocol or system is identified in the specifications. Please have the design team identify the controls to be used for the systems that incorporate zone dampers.
A5: We would recommend Prolon for any VVT controls, but we don’t have a spec or sequence for this as it is standard off the shelf equipment. This would be by the mechanical subcontractor with whichever equipment they submit.
Q6: The subcontractors for Data are saying they don’t see any specs for Data outlets and the legend calls for rough-in only. Could you clarify if we need to include Cat 6 installation and termination in our proposal?
A6: We are showing rough-in only, no CAT6 cabling or terminations. The cabling and terminations are done by KTHFS or KTHFS vendor for their systems.

3.2.2. Provide duct accessories of materials suited to duct materials; use galvanized-steel accessories in galvanized-steel, stainless-steel accessories in stainless-steel ducts, and aluminum accessories in aluminum ducts.
3.2.3. Install backdraft dampers on exhaust fans or exhaust ducts nearest to outside and where indicated.
3.2.4. Where installing volume dampers in ducts with liner; avoid damage to and erosion of duct liner.
3.2.5. Provide balancing dampers at points on supply, return, and exhaust systems where branches lead from larger ducts for air balancing. Install at a minimum of two duct widths from each branch takeoff. Provide balancing dampers for all air inlets and outlets.
3.2.6. Provide test holes at fan inlets and outlets and where required for air testing and balancing.
3.2.7. Install fire dampers, with fusible links, and in accordance with manufacturer's UL-approved written instructions.
3.2.8. Install all dampers square and free from racking with blade running horizontally.
3.2.9. Do not compress or stretch damper frames into duct or opening.
3.2.10. Handle dampers using sleeve or frame. Do not lift dampers using blades, actuators, or jack shafts.
3.2.11. Install bracing for multiple sections to support assembly weights and hold against system pressure. Install bracing as needed.
3.2.12. Turning Vanes:
3.2.12.1. Vanes must be installed in all square elbows, eliminating every other vane is not allowed.
3.2.12.2. Single thickness vanes cannot be over 36-inches long without intermediate vane runner.
3.2.12.3. Install per SMACNA and fasten/support to prevent vibration, noise, and to maintain proper alignment at design velocity.
3.2.13. Install duct access doors to allow for inspecting, adjusting, and maintaining accessories and terminal units as follows:
3.2.13.1. On both sides of duct coils.
3.2.13.2. Downstream from volume dampers and equipment.
3.2.13.3. Adjacent to fire dampers, providing access to reset or reinstall fusible links.
3.2.13.4. To interior of ducts for cleaning; before and after each change in direction, at maximum 50-foot (15-m) spacing.
3.2.13.5. Install the following sizes for duct-mounting, rectangular access doors:
3.2.13.5.1. One-Hand or Inspection Access: 8-inches by 5-inches.
3.2.13.5.2. Two-Hand Access: 12-inches by 6-inches.
3.2.13.5.3. Head and Hand Access: 18-inches by 10-inches.
3.2.13.5.4. Head and Shoulders Access: 21-inches by 14-inches.

3.2.13.7. Label access doors.
3.2.14. Install flexible connectors immediately adjacent to equipment in ducts associated with fans and rooftop units (RTU-1 through RTU-5, EF-1, EF-2) supported by vibration isolators. Provide sheet metal weather cover over flexible connections located outdoors.
Attach sheet metal to either equipment side or ductwork side, but not both.
3.2.15. For fans developing static pressures of 5-inch wg (1250 Pa) and higher, cover flexible connectors with loaded vinyl sheet held in place with metal straps.
3.2.16. Install duct test holes where indicated and required for testing and balancing purposes.
