About this file
This is Addendum #2 to RFP-KTHFS-2026-003 for a fitness center expansion project issued by Klamath Tribal Health and Family Services (KTHFS) in Chiloquin, Oregon. The project consists of a 20,000+ square foot addition to the existing fitness center that will include a community room, workout room, teaching kitchen, wellness center, smoothie bar, offices, restrooms, and showers. The addendum extends the final date for RFP questions to Thursday, October 1, 2026 at 12:00 p.m. and maintains the submission deadline of Wednesday, October 7, 2026. The addendum provides clarifications and updates to the original bidding documents through ten question-and-answer items and specification amendments, including revised electrical specifications confirming a 600kW unit with a new 2000-amp rated CT cabinet, open transition automatic transfer switch, and updated panel schedules. Multiple drawing sheets have been updated to reflect coordination changes, including reflected ceiling plans with added Big Ass Fans, electrical power plans with enlarged kitchen details, roof framing details with clarified outlooker sizing, and civil plans addressing demolition and grading coordination.
KTHFS will remove existing equipment and furnishings before work commences, while the owner will supply and install certain items including kitchen appliances, exercise equipment, sauna, cold plunge, restroom accessories, and health assessment equipment. The contractor must maintain all building exits during construction and coordinate operations with KTHFS Facilities/Security Director and Health Project Manager to minimize disruption to the adjacent occupied facility. Davis-Bacon prevailing wage rates apply per the latest published wage schedule, and removed excavated material will remain on-site with owner coordination. The owner will hire a third-party agency for special inspections and testing, removing cost burden from the contractor's allowance. Contractors must provide backing for wall-mounted toilet accessories and install contractor-furnished items including grab bars, mirrors, towel bars, shower curtain rods, baby changers, hooks, and recessed wastebaskets, while KTHFS will supply and have a vendor install dispensers for toilet paper, paper towels, soap, seat covers, and feminine hygiene products.

ADDENDUM NO. 002
TO ALL PLAN HOLDERS:
This Addendum forms a part of the Contract Documents and modifies the original bidding documents released for bid preparation. Please notify everyone concerned as your sub-contractors and suppliers of the issuance and contents of this addendum prior to date of bid opening.
THE FINAL DATE RFP QUESTIONS WILL BE ACCEPTED IS THURSDAY, OCTOBER 1, 2026, AT 12:00 P.M.
QUESTIONS AND CLARIFICATIONS:
Q1: We still need to know if the ATS switch will be a 3-pole or 4-pole unit. Is the neutral switched or not?
A1: Three Pole.
Q2: Can you check with the electrical engineer if a closed transition ATS is needed? We typically only see these in data centers, I believe it may be a carryover specification.
A2: Open Transitions ATS.
Q3: The plans show 600kw, not 500kw. Are the plans going to be updated with an addendum, so everyone is quoting the correct unit?
A3: 600kw Unit.
Q4: It was discovered today that the existing CT/Meter shown to be reused for the new service on sheet E401 will not work because the equipment is only rated for 1,200 amps. The new service is scheduled to be 2,000 amps and cannot be passed through equipment only rated for 1,200 amps. Please review and update the sheet E401 so the gear manufacturers can quote the project.
A4: See revised Sheet E401.
Q5: There is not a site demolition plan. Please provide one identifying removals and limits for existing pavement, sidewalks, curbs, utilities, landscaping, trees, irrigation, and other site improvements.
Identify items to remain, items to relocate, and required restoration.
A5: See civil sheet C010 for extent of work of project. Demolition of items to complete intent of the work. Trees are to be removed within the building footprint and 10’ away from building and exterior amenities.
Q6: Please confirm the base-bid excavation limits and depths, including the recommended 3-foot removal, 5-foot extension beyond the building, and geogrid requirements. How will additional unsuitable material removal or additional geogrid directed during construction be measured and paid?
Can excavated material remain onsite, or is offsite disposal required?
A6: This is in the exhibits; removed material will be on-site disposal. Coordinate with owner.
Q7: Please provide the required excavation and protection details where new work adjoins existing foundations. Clarify responsibility for additional shoring or underpinning if the existing foundation or reinforced fill pad differs from the assumed conditions.
A7: Means and Methods. Contractor’s responsibility to protect existing construction and area outside of scope of work
RFP-KTHFS-2026-003 – ADDENDUM #2 – Chiloquin Fitness Center Expansion Project Page 6
Q8: Please identify an approved discharge location for construction dewatering and any treatment or permitting requirements.
A8: Owner confirmed existing settling ponds can be used.
Q9: Please provide the governing Davis-Bacon wage determination and revision, and confirm whether Oregon BOLI rates will also apply. The contractors indicated this was in the documentation supplied by your firm A9: Davis-Bacon wage determination is published; refer to website. Utilize latest wage schedule at the time of bid.
Q10: Please provide a coordinated equipment schedule identifying who furnishes, unloads, installs, connects, and starts up each item. Resolve the overlapping owner-install and contractor-install requirements for kitchen equipment, exhaust hoods, sauna, cold plunge, and fitness equipment. This appears to be vague in the drawings.
A10: Contractor and/or Owner provided items clarified in specifications – 01 10 00 Summary Addendum #2 and 10 28 00 Toilet, Bath and Custodial Accessories Addendum #2. Provided new specification sections for Chiloquin and Beatty.
Q11: Please verify if R-15 or R-10 slab-edge insulation is required?
A11: R-10 vertical insulation.
Q12: Please provide the testing allowance amount referenced in the specifications and identify testing and inspections paid directly by the owner. We'd recommend this be pulled out and inspection company be hired directly by Owner.
A12: Owner will hire a third-party agency for special inspections and testing required during construction. Specification 01 40 00. Section 1.07 Part A. REMOVE: “Payment for cost services will be derived from allowance specified in Section 01 21 00; see Section 01 21 00 and applicable sections for description of services included in allowance.”

506.3 - Frontage increase: Every building shall adjoin or have access to a public way to receive an area factor increase based on frontage. Area factor increase shall be determined in accordance with Sections 506.3.1 through 506.3.3.
Area Frontage Calculations:
Total building perimeter (Expansion & Existing) = 403 Linear Feet Percentage of building perimeter having min 20' public way or open space = 100% Frontage Increase Factor (75%-100% of building perimeter is 30'+ distance) = 0.75 Allowable Area = 36,000 SF + (9,000 x 0.75= 6,750 SF) = 42,750 SF (B Occupancy)
24,000 SF + (6,000 x 0.75= 4,500 SF) = 28,500 SF (A-3/A-4 Occupancy)
Existing Building GSF (15,152 SF) + Proposed Expansion GSF (20,319 SF)
508.3.3 - Allowable building area, height, and number of stories: The allowable building area, height, and number of stories of the building shall be based on the most restrictive allowances for the occupancy groups under consideration for the type of construction of the building.
Table 508.4 - Required Separation of Occupancies A-3 Occupancy to B Occupancy = 2 Hour (w/Fire Sprinklers)
Addition + Existing B Occupancy = 11,109 SF (Addition) and 4,944 (Existing) A-3 Occupancy = 9,210 SF (Addition) A-4 Occupancy = 10,208 (Existing)
