About this file

Addendum #3 is a modification to the original bidding documents for RFP-KTHFS-2026-003, issued by Klamath Tribal Health and Family Services (KTHFS) for the Chiloquin Fitness Center Expansion Project in Chiloquin, Oregon. The project consists of construction of a 20,000+ square foot addition to the existing KTHFS Fitness Center, including a community room, workout room, teaching kitchen, wellness center, smoothie bar, additional offices, restrooms, and showers. The final date for RFP questions is Thursday, October 1, 2026, at 12:00 p.m., with a submission deadline of Wednesday, October 7, 2026. Addendum #3 provides clarifications and updates to specifications and drawings, including revisions to Section 11 30 13 (Appliances), mechanical schedule changes specifying Daikin equipment instead of Trane, updated electrical disconnect sizes and breaker configurations on drawing E202, and revised panel schedules on drawing E501. Key questions addressed include AWI QCP certification requirements (not required for project registration), inspection scope (no inspection required), permitted cable types (both MC and Romex allowed with Romex restricted to concealed walls only), door specifications in the workout area (two 10' x 9' overhead sectional garage doors with specification section 08 36 13), and signage responsibility (Owner-Provided, Contractor-Installed with coordination required on types, locations, and quantities).

The project establishes clear delineation between contractor-furnished and owner-furnished equipment. KTHFS will supply all appliances, furnishings, furniture, exercise equipment, kitchen appliances, restroom accessories, and other non-structural items for installation by contractors, while the contractor is responsible for furnishing and installing specific items including a 1/2 HP GE continuous-feed waste disposer (Model GFC525N), GE 30" wall-mount range hood (Model PCST22U1WSS), Demonstration Mirrors (Mirrorlite 48" x 72" x 1.25", quantity two for the training kitchen), cooking exhaust systems, and all related plumbing and electrical connections. KTHFS will continue occupying adjacent portions of the existing building during construction, requiring contractors to maintain all emergency building exits, limit utility outages to unoccupied hours, provide seven business days notice for life safety system disruptions, and coordinate all work with KTHFS Facilities/Security Director and Health Project Manager. Mechanical equipment includes five Daikin rooftop units (RTU-1 through RTU-5) with specifications detailed in the revised schedules, and all HVAC systems must comply with ASHRAE 90.1-2022 energy code requirements with demand control ventilation, supply and return smoke detectors, and single-zone VAV controls.

ADDENDUM NO. 003

TO ALL PLAN HOLDERS:

This Addendum forms a part of the Contract Documents and modifies the original bidding documents released for bid preparation. Please notify everyone concerned as your sub-contractors and suppliers of the issuance and contents of this addendum prior to date of bid opening.

THE FINAL DATE RFP QUESTIONS WILL BE ACCEPTED IS THURSDAY, OCTOBER 1, 2026, AT 12:00 P.M.

QUESTIONS AND CLARIFICATIONS:

Q1: Project Certification – Is this project itself required to be registered and issued an AWI QCP Certificate of Compliance, or is the requirement solely for the millwork manufacturer / installer to hold current AWI QCP Certification?

A1: It is not required for the project to be registered.

Q2: Inspection Scope – Please confirm if AWI QCP installation inspection services are required to be contracted for this specific scope or its self-certification / compliance documentation by an AWI QCP certified shop is sufficient.

A2: No inspection is required.

Q3: Can you please clarify if MC Cable and Romex cables are permitted for branch circuits? Section 26 05 19 2.01K seems to allow MC, but Section 2.05 says only with written approval. Romex is not mentioned in those sections, but 26 05 29 3.01E seems to allow the use of Romex.

A3: Both MC and Romex cables are good to use. Romex cables can only be used if they are concealed within the walls; Romex cables cannot be put above any dropped or suspended ceilings.

Q4: I see there are 2 doors in the workout area, and maybe a counter door. These doors are not listed in the door schedule. Do you know if there will be another addendum that will have these listed and if they will have a spec section to follow?

A4: All doors in the workout area have tags included on the Floor Plan, A111. Doors 107D and 107E are the overhead sectional garage doors, 10’ wide x 9’ tall. Spec section was included, refer to Section 08 36 13 – Sectional Doors.

Q5: I found accessibility sign requirements and room names, but no architectural sign schedule or sign locations. Please confirm which room identification, restroom, directional, and exterior signs are in this bid, and provide sign types, sizes, finishes, copy, and quantities. Please also confirm whether AED and fire extinguisher cabinet markings are included with the cabinets, and whether MEP identification labels are being carried by those trades. I can price the signage once that scope is defined A5: Signage will be Owner Provided, Contractor Installed. Coordinate with owner on which rooms are to have signage, signage type, locations, etc.
