Addendum 2 Summary

Addendum 2 is a supplemental document issued by the Florida Department of Transportation (FDOT) District Three in connection with the District 3 Midway Operations Center Supplemental Crew procurement. This addendum modifies or clarifies the original Request for Proposal for staffing and equipment to provide routine maintenance services on roadways in Leon and Gadsden Counties, with operations based at the Midway Operations Center located at 17 Commerce Blvd, Midway, FL 32343. The solicitation requires a minimum of one crew consisting of one crew leader, four maintenance workers, and one equipment operator to perform asphalt repair, drainage infrastructure maintenance, roadside vegetation management, traffic services, and emergency response activities. Technical proposals with a maximum of eight pages must be submitted by April 9, 2026 at 10:00 AM via email to D3.Purch@dot.state.fl.us, with public opening of technical proposals at 11:00 AM the same day at 1074 Highway 90 East, Chipley, FL 32428. Price proposals will be opened separately on April 23, 2026, and the selection meeting is scheduled for May 4, 2026. The contract term is anticipated to begin July 1, 2026 and continue through June 30, 2027, with renewal options for up to three additional one-year periods contingent upon satisfactory performance and fund availability.

Evaluation will be based on a 70-point technical score addressing philosophy, qualifications, past performance, safety, Maintenance & Repair Program strategy, added value, and proposer-selected topics, combined with a 30-point price score, with award to the responsive and responsible vendor achieving the highest cumulative score. The pricing structure requires submission of hourly rates for crew members and equipment based on estimated annual hours, with a fixed $10,000 equipment contingency included as a non-bid line item. The total contract budgetary ceiling is $500,000, and the contract will be awarded based on this budgetary amount rather than the proposal amount submitted. All proposers must be registered in MyFloridaMarketPlace, comply with E-Verify requirements, and carry general liability insurance with combined bodily injury limits of at least $200,000 per person and $300,000 each occurrence, plus property damage insurance of at least $200,000 per occurrence. Contractors must also provide a performance bond in the full amount of the proposal. Monthly payments will be made upon submission of invoices with supporting documentation, subject to a one percent MyFloridaMarketPlace transaction fee.
