This conduit is the pathway to be followed for the fiber from the MDF to IDF.

Page 59 of 64 – Section L.13 GENERAL INSTRUCTIONS, Subsection (i) General, paragraph (B) states:

“Any experience provided for a firm other than the offeror(s) (prime contractor(s)) that is not listed under their name/SAM Unique Entity Identifier (UEI) number submitted under this solicitation will not be considered.”

Question: This statement is repeated in Section M, Evaluation Factors for Award. It seems to imply that ALL project examples provided in the proposal by the Prime Contractor must have the same UEI number as the office location from which the proposal will be submitted.

Is this correct? If so, this is extraordinarily restrictive, especially for small businesses. Small companies with multiple offices may have performed projects in different locations that are relevant to the solicitation criteria, but they could not be presented in this proposal if they were not performed by the office submitting the proposal.

Please clarify.

Britton Reference Amendment A00001, revised clauses L.13. GENERAL INSTRUCTIONS, Subsection (1)(i) General, paragraph (B) and M.1 EVALUATION FACTORS FOR AWARD paragraph (g).

Specialized Experience

Page 59 of 64 – FACTOR 1 Specialized Experience, paragraph

(iii) states:

“If any experience will be used for evaluation of other than the prime firm, a teaming arrangement shall be provided….”

Question: This seems to be in direct conflict with the statement made in the GENERAL INSTRUCTIONS, Subsection

(i) General, paragraph (B).

Please clarify whether the project experience of a subcontractor/teaming partner will be acceptable, and if so, how their UEI would be evaluated.

Also, if subcontractor/teaming partner experience will be acceptable, will their projects count toward the maximum of five projects?

Britton Reference Amendment A00001 for revised clause

L.13. GENERAL INSTRUCTIONS.

Yes, the subcontractors experience would count towards the max projects.

19. FACTOR 2

Past Performan ce

Page 60 of 64 – FACTOR 2 Past Performance – regarding Past Performance Questionnaires.

Question: We understand that a PPQ is only required if there is no CPARS on record. If we have a previously received PPQ, will it be acceptable to present that in the proposal in lieu of a new one?

Clients move around and it is sometimes difficult to identify an individual familiar with the project after a few years.

Britton Yes, as that’s the intent of the PPQ form, to be used over and over.

20. Please provide the water pressure and available supply volume for the facility.

Groh Hualapai Fire and EMS provide hydrant flow tests for hydrants near the clinic, included in Attachment J01j – Hydrant Flow Tests.

21. If the water pressure and/or supply volume are not adequate to support a wet fire sprinkler system, would IHS be opposed to repairing the existing dry system?

Groh IHS does not want a dry system. Even if a fire pump is necessary to maintain pressure, a wet-pipe sprinkler system is the preference.

22. If the communication lines are removed from the existing building how will it impact the temporary facilities? Are the communication lines for the trailers tied to the existing building?

Groh The communication lines that are impacted are those lines going to specific wall boxes.

Communication lines between buildings and to the utility are not anticipated to be impacted.

Amendment A00002 dated 06/06/2023

23. The proposed location of the dental swing space is South of the existing trailers. There is not enough elevation difference to facilitate drainage into the existing sewer manhole. We propose setting the dental swing space to the East of the designated area so that the facility can sit at a higher elevation. A concrete walkway can be added between the trailer and the asphalted surface. Is this acceptable?

Groh The IHS lease ends at the pavement. We cannot move the building east, off the IHS leased land.

24. What are the amperage requirements for the dental equipment?

Groh The relevant pages from the equipment installation guides are provided in Attachment J01jk – Dental Equipment Electrical Requirements.
