PROGRAM - Fill in program acronym and class number (i.e., CITP-701, UPTP-701, etc.)
CLASS COORDINATOR - Fill in the name of the class coordinator responsible for this class
PROGRAM TECHNICIAN - Fill in the name of the Training Technician responsible for this class
EDUCATIONAL AIDE - Fill in the name of the Educational Aide responsible for this class
DATES CONDUCTED - List the inclusive dates of the class (i.e., March 21-June 2, 2007)
SVC/COTR for ED AIDES - Fill in the name of the Student Services COTR

Name, signature and date of approvals for the positions indicated.

Agency - List the agency acronym for each agency participating in the class
Enrolled - The number of students from the agency beginning the class
Graduated - The number of students from the agency that graduated
TOTAL - Total the columns
GRADE POINT AVERAGE OF CLASS - List the grade average for the class
EDUCATIONAL AIDE COMMENTS - Comments from the Educational Aides (Optional)
Academic/PE Failures - The number of students from the agency that were removed from training for failing to meet the evaluation requirements of a graded exercise, written examination or graded PE.
Non-Academic Withdrawals - The number of students from the agency that were removed from the class for cheating, discipline issues, medical reasons, voluntary withdrawals, agency removal, or other non-academic reasons.
