Objectives To review the entire Contractor Screening packet forms
To establish clear criteria for Contractors submitting badge application forms
Improve efficiency in the badging process by reducing errors and incompleteness
DHS Continuous Vetting Notification
• New guidance has been issued from DHS Headquarters and now includes enrollment into a Continuous Vetting Program for Tier 1 and Tier 2 Contractors.
• Temp Intermittent Contractors are exempt from this requirement.
• Automated checks may reduce the periodicity of reinvestigations.
Please ensure that every Contractor is provided a copy to read/review
DHS Contractor Fitness/Security Screening Request Form DHS 11000-
• Section I, II, and III filled out completely.

• Form version is current (7/17) and all 4 corners of the form are legibly visible
• Full Legal name of applicant
• Date of Birth and Social Security numbers matching on all forms and Identification

Low Risk 1N = SAC (FLETC Center access only - No Network or building access needed.)
Low Risk 1N = Tier 1 (FLETC center and building access – No Network access) Moderate Risk 5N= Tier 2
Please do not use Solicitation numbers. Until a contract is awarded, we cannot process any badge applications.
Badge applications are only accepted from the CO, COR, or Federal Sponsor once request form has been completed. Contract companies submitting directly to Security Staff will be rejected.

• #5 indicate if Contractor requires access to FLETC Network

Dates of Access will be the dates needed, not to exceed the end dates of the current contract and not to exceed 1 year
This question is what determines if a Moderate Risk position receives a PIV card or a 1-year Proximity card.

• Any corrections to this form, require the Applicant’s Full Initials and date the corrections made next to ALL corrections.

• E-Verify reports can be submitted in place of the USCIS Form I-9.
• Employee must sign and Employer who verified the Identification documents

Selection of responses 2-4 will require an additional form to be filled out. The DHS Form 11055 Foreign National Screening Request Form.(See slide #12)
