About this file
This is a submittal requirement document for the Broward Sheriff's Office (BSO) pre-qualified list of IT consultant services solicitation in Florida. The document establishes mandatory disclosure requirements for proposers regarding litigation history and regulatory actions. Proposers must provide detailed information on all current litigation, judgments, settlements, and court cases related to providing the IT consulting services outlined in the solicitation for the past three years, with bankruptcy cases requiring disclosure for the past seven years. The definition of "case" encompasses lawsuits, criminal cases, bankruptcy filings, assignments to creditors, administrative actions, and arbitrations. For proposed joint ventures or team arrangements, each member must submit requested information separately.
Proposers are required to disclose litigation and regulatory actions by type, identifying the court or regulatory agency, case or file number, and current status or disposition. Proposers and their principals must also indicate any debarment or suspension from conducting business with government agencies or professional boards. The document emphasizes that proposers maintain an ongoing obligation to update this form throughout the solicitation process and until contract execution, with notification to the BSO Purchasing Agent of any litigation initiated or reopened during the solicitation period. Failure to maintain an updated case list may result in automatic disqualification or termination of an award. "Principals" are defined to include corporate officers and controlling shareholders, managing partners and capital owners in partnerships, and parent or subsidiary entities, depending on the organizational structure.
