About this file

This is a Responsible Vendor Review Form from the City of Tallahassee, Florida, required for vendors responding to the Senior Center Roof Project solicitation. The form establishes the criteria and standards by which the City determines vendor responsibility and capability to perform contract obligations. Vendors must complete a comprehensive questionnaire addressing potential disqualifying factors, organizational capability, and relevant experience. The City's responsibility determination is based on information provided by the vendor in the form and solicitation response, as well as independent research conducted by the City, including verification through the System for Award Management (SAM). Vendors are required to provide immediate written notice to the City if any information submitted becomes erroneous prior to contract execution.

The form requires vendors to disclose any adverse actions within the previous ten years in Florida or any state or federal jurisdiction, including permit revocations, suspensions, disbarments, government contract suspensions or debarments, formal monitoring agreements, indictments or civil actions related to government contracts, contract terminations for cause, criminal convictions related to contracting, governmental investigations, and civil fraud judgments. Vendors must also affirm they possess the necessary organization, experience, accounting controls, operational controls, and professional and technical skills to meet contract obligations. Additionally, vendors must indicate whether they maintain an active State Term Contract (STC) or General Services Administration (GSA) contract similar to the project's scope of services. Answering affirmatively to any adverse action question does not automatically disqualify a vendor but requires detailed explanation and supporting documentation; failure to provide requested information may result in disqualification.
