About this file

This is an evaluation score sheet for a Request for Proposals issued by the Utah State Board of Education (USBE) for Utah Professional Practices Advisory Committee (UPPAC) Investigators under Solicitation Number USBE-MI27022-RFP. The RFP seeks to contract with qualified external, independent investigators who will be assigned on a rotating or random basis as hearings are required. The resulting contract is anticipated to run through June 30, 2027, with potential for four additional one-year renewal options. USBE reserves the right to terminate the contract at any time in accordance with standard terms and conditions.

The evaluation scoresheet uses a 0-10 point scale to assess proposals across technical and cost criteria. Technical evaluation comprises 71 percent of total points (250 points possible), divided into four weighted sections: Section 1 (40 percent, 100 points), Section 2 (28 percent, 70 points), Section 3 (20 percent, 50 points), and Section 4 (12 percent, 30 points). Cost evaluation comprises 29 percent of total points (100 points possible), calculated using a formula where the lowest-cost proposer receives 100 percent of price points and other proposers receive points based on the percentage difference from the lowest cost, with proposers exceeding 200 percent of the lowest cost receiving zero points. A minimum score of 162.5 points (65 percent of technical points) and 227.5 points overall (65 percent of total points) is required to pass evaluation. Scoreable criteria include potential conflicts of interest, educational law experience, investigator experience, writing samples, data protection protocols, and representation of educators facing UPPAC discipline.

* USBE will use the following cost formula: The points assigned to each offerors cost proposal will be based on the lowest total cost for the duration of the contract. The offeror with the Lowest Total Cost will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Total Cost is than the Lowest Total Cost. An offeror whose Proposed Total Cost is more than double (200%) the Lowest Total Cost will receive no points. The formula to compute the points is: Cost Points x (2- Proposed Total Cost/Lowest Total Cost).
