About this file
This is a vendor questionnaire related to an audiovisual (AV) system installation and support contract. The document outlines evaluation criteria across three primary categories: vendor experience and qualifications, project management and approach, and training, support, and warranty provisions. Vendors must demonstrate at least three years of business operation and provide documentation of three comparable projects completed within the past three years. The AV system design must meet all functional requirements specified in the scope of work, and vendors must submit detailed user interface specifications, project timelines covering design through training phases, and comprehensive documentation including drawings, signal flow diagrams, and ceiling reflection plans. Installation quality standards, safety protocols, subcontractor qualifications, and scope change management procedures are required elements of vendor proposals.
The questionnaire requires vendors to detail their training programs for end-users, post-installation support capabilities, and warranty/service level agreements. Documentation deliverables must include user guides and as-built drawings. Vendors must confirm compliance with all federal, state, and local regulations and affirm adherence to scope of work requirements, contract terms and conditions, billing procedures, and insurance requirements, including submission of a Certificate of Insurance per contractual specifications. The questionnaire serves as an evaluation instrument to assess vendor capability to successfully design, install, commission, and provide ongoing support for the AV system project.
