About this file

This is a Contract Modification #2 and Extension #1 document between the Franklin County Board of Commissioners and Metropolitan Life Insurance Company for the continuation of voluntary group disability insurance and related benefits for the Franklin County Cooperative Health Improvement Program. The extension modifies the original contract entered into on January 18, 2022, under RFP #2020-06-35, with a subsequent first amendment executed on December 18, 2023, to add group legal services plan benefits. The extension term runs from January 1, 2025, through December 31, 2026, representing a 24-month extension period with no additional renewal options remaining. The services include voluntary short-term and long-term disability insurance, group legal services, and the addition of pet insurance and potential identity theft insurance coverage for benefits-eligible employees.

The total not-to-exceed amount for the extension is $185,000.00. For voluntary long-term disability, renewal rates are guaranteed through December 31, 2026, with a 1.1 percent decrease across all age bands, generating annual premiums ranging from $29,483 to $259,327 depending on age group. Voluntary short-term disability rates show a 4.2 percent reduction with annual premiums between $60,605 and $276,662. The employee-paid group legal services plan participation fee remains at $16.50 per month, while an employer-paid participation fee of $8.25 per month is newly added. Pet insurance is offered as an entirely employee-paid benefit with rates determined by plan selection. Identity theft insurance rates, if adopted, range from $3.00 to $7.85 per employee per month for employer-paid options and $5.55 to $12.95 for voluntary options. The contract notice contact for Metropolitan Life Insurance Company is Charlie Cross, Senior Account Executive, located in Southfield, Michigan.
