Cost Proposal Form Summary

This is a Cost Proposal Form for the Utah State Board of Education (USBE) Request for Proposals Solicitation #USBE-SD26147-RFP for Utah Driver Education Online Scheduling Software. The solicitation seeks to establish cooperative contracts with qualified offerors to provide driver education online scheduling software that enables customers to register for classes, sign custom contracts, make payments, schedule drive times, receive text message reminders, and access teacher feedback. The RFP anticipates multiple contract awards for solutions meeting specified requirements. The contract term spans five years (Years 1-5), with pricing adjustments in subsequent years limited to a maximum of 102% of the previous year's cost.

The cost proposal form requires offerors to submit maximum licensing fees for five license size categories: Mini (1-50 users), Small (51-150 users), Medium (151-500 users), Large (501-3,000 users), and Extra-Large (3,001+ users), with Extra-Large fees capped at 1.5 times the Large license fees. Category 1 licensing fees account for 90% of the total cost points (300 points possible), weighted as follows: Mini at 10%, Small at 15%, Medium at 20%, Large at 44%, and Extra-Large at 1%. Category 2 requires offerors to identify and cost any other mandatory costs for using the proposed solution or meeting RFP requirements, comprising 10% of total cost points (30 points possible). Scoring is based on the lowest average total cost per year, with the lowest-cost offeror receiving 100% of points and other offerors receiving proportional points based on cost variance. All mathematical calculations must be accurate, and any deviation from the prescribed format may result in proposal disqualification.

The points assigned to each offeror's cost proposal for each item (i.e., license size and other mandatory costs) will be based on the lowest average total cost per year. The offeror with the lowest average total cost per year will receive 100% of the price points for that item. All other offerors will receive a portion of the total cost points for that item based on what percentage higher their average total cost per year is than the lowest average total cost per year.
