About this file

This is a Contract Modification #2 and Extension #1 document between the Franklin County Board of Commissioners and Metropolitan Life Insurance Company, executed to extend and modify an existing voluntary group disability insurance and benefits contract. The original contract was executed on January 18, 2022, under RFP #2020-06-35, with a first amendment added on December 18, 2023, to include group legal services plan benefits. The extension establishes a new term running from January 1, 2025, through December 31, 2026, representing an additional 24-month period with no further extension options remaining. The contract covers voluntary group short-term and long-term disability insurance for Franklin County Cooperative Health Improvement Program members and adds new benefit offerings including employer-paid group legal services, pet insurance, and optional identity theft insurance coverage.

The total not-to-exceed amount for the extension period is $185,000.00. Renewal rates for long-term disability show a 1.1 percent decrease across all age bands, with annual premiums ranging from $29,483 to $277,279 depending on age and covered payroll volume. Short-term disability renewal rates decrease 4.2 percent, with annual premiums ranging from $60,605 to $276,662. The employee-paid group legal services participation fee remains at $16.50 per month, while a new employer-paid legal services option is added at $8.25 per month. Pet insurance is offered as an entirely employee-paid benefit with rates determined by individual plan selection. Identity theft insurance rates, if adopted by the County, range from $3.00 to $7.85 per employee per month for employer-paid options and $5.55 to $12.95 for voluntary employee-paid options, guaranteed for three years. Metropolitan Life Insurance Company serves as the incumbent contractor, with notice contact information updated to Charlie Cross, Senior Account Executive, in Southfield, Michigan.
