About this file
This is Amendment 3 to RFP 2025-06-09 issued by Franklin County, Ohio, Purchasing Department, on September 30, 2025, regarding Voluntary Programs including Short and Long Term Disability, Legal Plan, Identity Theft, and Pet Insurance. The amendment documents the closure of the question submittal period and provides comprehensive written responses to 40 vendor questions addressing product specifications, eligibility requirements, enrollment procedures, claims administration, and operational details. Key program parameters include post-tax premium contributions for both STD and LTD; participation through the Ohio Public Employees Retirement System (OPERS) for most employees and the Ohio Police & Fire Pension Fund (OP&F) for certain police and fire personnel; participation thresholds measured at the Cooperative aggregate level for the 10% payroll deduction trigger; a legal plan network requirement covering Franklin County and six adjacent counties (Delaware, Fairfield, Licking, Madison, Pickaway, and Union) with 48-72 hour appointment availability; and current legal plan participation at approximately 30.6% of eligible employees.
Vendors may submit proposals for any combination of the five requested products, with missing lines of coverage impacting scoring; subcontracting is permitted provided the bidding vendor holds and administers the contract. The County uses the WEX benefits administration platform and does not pay commissions; all pricing must be submitted net of any commissions. Pricing should reflect salary bands and age ranges at midpoint values, as detailed census information with individual salaries and dates of birth will not be provided. The County is considering scenarios for both employee-paid and employer-paid STD coverage for all employees, distinct from current individual agency arrangements. Vendors should submit their best and lowest offers without expectation of additional negotiation, and implementation or technology credits offered must represent best offers. The request for proposal includes historical experience data covering three to five years, including earned premiums, insured lives, rate history, paid claims, and open claim details for disability products.

AMENDMENT 3
Please note the following changes have been made to the RFP packet.
I. Appendix 4.8 RFP Question Submittal Form
The question submittal period is now over and no further questions regarding the RFP document will be accepted by the County. The following questions were submitted by vendors and answered by the County in writing:
Questions and Answers:
Q# Appendix or RFP Document:
Section Number: Question: Answer:
1. RFP Are there any known billing, service or admin related issues causing this opportunity to go to market? If yes, please elaborate?
The County is required to look at the market via the RFP process on a regular basis. That timing is the prompt for this RFP.
2. RFP Please confirm if this bid is being requested net of commission?
The County does not pay commissions. Please show pricing net of any commissions.
3. RFP Please confirm if it’s a requirement to quote all lines of coverage requested on the bid, or if carriers will be considered who cannot offer all products.
Vendors may submit a proposal with any combination of lines of coverage. However, all requested lines of coverage are being scored. Missing lines of coverage in the proposal will impact scoring.
Vendors may submit bids with subcontracted partners for lines of business they do not directly provide. The bidding vendor must hold the contract and administer the relationship with the subcontracted vendor.
4. STD Is W2 prep included in STD coverage? Vendors are expected to provide W2 prep when there is tax liability. Any additional pricing for this service should be included on the pricing document.
5. All products What is the Benefit Administration Platform you are using for benefits?
WEX
6. Legal What is the current participation rate for the legal plan?
Approximately 30.6 % of employees who are currently benefit eligible are enrolled in the legal plan.
7. RFP Question STD: Please confirm whether the group participates in Social Security and if so, if any occupations are exempt.
Employees participate in OPERS (Ohio Public Employees Retirement System) through employment with the County.
Certain employees of Jefferson, Prairie and Grandview Heights (police and fire) participate in OP & F (Ohio Police & Fire Pension Fund).
8. RFP Question STD: Please confirm whether the premium contributions are paid with pre or post tax dollars.
Post tax
9. RFP Question STD: Please confirm whether the group prepares W-2s for STD claimants, or if the group requires the carrier to do so.
The carrier is required to prepare the W-2’s in cases of tax liability. Any additional pricing for this service should be included on the pricing document.
10. RFP Question STD: Please confirm whether the group currently has telephonic claims service on the STD.
Based on the County’s understanding of this question, employees can initiate a claim, obtain claim updates and request member services assistance telephonically. A mobile app is also available which allows employees to interact in the claim process.
11. RFP Question STD: Please clarify if the group offers other STD plans to employees or if there are any other short-term disability plans with payroll deductions.
a. If so, please indicate whether the group will eliminate these additional policies if a new STD carrier is selected.
The Cooperative does not offer other STD plans that are payroll deducted to our employees. As noted in Appendix 4.11.b, one agency (8200) offers employer paid STD coverage in lieu of the Cooperative’s employee paid STD, and one agency (7000) has an individual agency policy that is employer paid and provides STD coverage after one year of employment which could exist alongside of the Cooperative’s employee paid STD program.
Another agency (9997) offers the Cooperative Plan STD coverage as employer paid coverage.

17. Census Can a census with zip codes please be provided?
5-digit zip code is provided in Appendix 4.11.a in column F labeled “PostalCode”
18. RFP Section 1.2; 3.0 The Cooperative includes 40+ agencies with their own HR processes. For eligibility, enrollment, billing, and reporting, should we treat each agency as a separate sub-account, or as one consolidated account with agency-level reporting?
Based on the County’s understanding of this question:
Eligibility is applied at the Cooperative level. Enrollment is reported by agency.
For billing/premium remittance outliers please see Appendix 4.11.b.
Reporting must be available at both the aggregate and agency-level.
19. RFP-Program Admin
Section 3.0 The 10% participation rule triggers payroll deduction vs. direct bill. Is the 10% threshold measured at (a) the Cooperative aggregate level, (b) each agency, or (c) each program by agency?
It is measured at the Cooperative level.
20. RFP-Program Admin
Section 3.0 The RFP requests a robust attorney panel in Franklin and adjacent counties with appointment availability in 48–72 hours.
Please confirm target counties and whether virtual appointments satisfy the requirement.
The adjacent counties are Delaware, Fairfield, Licking, Madison, Pickaway, and Union County.
Virtual appointments would satisfy the requirement if that was the members choice, not if they were forced to a virtual network.
21. RFP-
Communications 4
Communications You request co-branding and cross-promotion with other vendor partners.
Please identify the partners and channels (email, web portal, wellness newsletters) where cross-promotion is expected for Legal and ID Theft.
Please refer to page 4 of the RFP Document for a list of the Cooperative’s other vendor partners. Any opportunity for cross-promotion is encouraged. For example, on the topic of mental health and pets, cross promotion of pet insurance with the County’s EAP/BH vendor may be requested.
Channels (which include Cooperative-initiated as well as vendor-initiated) include all those named – email, web, newsletters – in addition to Open enrollment materials and communications.
22. RFP- Evaluation Criteria Please share weighting or relative importance among cost, implementation, account management, reporting, security, and member experience for Legal and ID Theft so we can tailor our responses.
The County does not publish weighting of RFP components. Your initial submission will be the basis of evaluation; therefore, it is recommended that your proposal contain your best and lowest offers.
23. RFP. Will Franklin County be releasing an updated census that includes salaries and dates of birth?
No. Salary bands and age ranges should be sufficient for rating purposes. Please assume the midpoint for each range.
24. RFP. Do individuals participate in PERS/STRS AND Social Security?
Employees participate in OPERS (Ohio Public Employees Retirement System) through employment with the County.
Certain employees of Jefferson, Prairie and Grandview Heights (police and fire) participate in OP & F (Ohio Police & Fire Pension Fund).
25. RFP. What should be included for commissions?
Refer to question 2.
26. RFP. The RFP states all plans are voluntary, with alternative non-contributory options being requested for consideration.
However, section 1.2 Organizational Background states ‘A handful of agencies purchase these coverages on behalf of their employees. Can you please obtain a full list of which entities are providing coverage as employer paid and a census identifying these members and which plans they are enrolled in?
Please refer to Appendix 4.11.b.
27. RFP For the alternative non-contributory plans requested below; are these for all EEs or just the 132 enrolled in ER Paid
STD?
The Employer paid quote is requested to consider a scenario where all employees would receive employer paid
STD.
At this time all EE’s rates are reflective of the regular rate, not an Employer paid rate.
28. Census Please provide updated census information with individual DOB and Salary for all members.
Refer to Question 24
29. Census Please provide an updated census that includes annual salaries and date of birth, rather than ranges.
Refer to question 24
30. RFP Please confirm if there were any rate or plan changes prior to 1/1/25?
Yes.
31. RFP Please confirm if there are 2 STD plans?
One that is Employee paid and another that is Employer paid? Or was that just an option you were considering for the 2025 renewal
Please refer to questions 27 & 28.
32. RFP If there is also an Employer-paid STD plan, who is eligible for the plan?
Please refer to Question 27 & 28.
33. RFP According to the RFP, MetLIfe currently provides the benefits and services you are looking for. At the bottom of page 3, it mentions it is your intent to have all voluntary programs contracted with one provider.
1. Do “voluntary programs” in this instance only apply to the insurance portion:
STD/LTD, Basic Life and Supplemental or to the other value adds as well?
“Voluntary programs” refers to our preference to have all 5 products, (STD, LTD, Legal, Pet, and ID Theft) with the same vendor.
34. At the top of page 4. It appears you encourage cooperative programming and partnering between vendors. Does this mean the following:
1. If a prime vendor subcontracted with other to provide you the completed suite of offerings the submission would be acceptable and not rejected? This is provided the broker/company was responsible for the management of the offering not Franklin County.
2. If a prime only submitted for a portion of your request, (i.e., voluntary insurance benefits) the submission considered?
Q 35.1 Yes
Q 35.2 Yes
For more information refer to question 3.
35. Census Can DOB, Salary & Class indicators be added to the census?
Refer to Question 24.
36. Commissions What commissions should be included on STD/LTD if any?
Refer to question 2.
37. Credits Are any specific tech or implementation credit amounts being requested?
The County has no specific amount they are expecting to be offered.
If your firm is offering a tech or implementation credit it should be your best offer as there will be no additional opportunity during the bid process to improve your offer.
38. EAP Are any EAP services currently in place or being requested? If so what coverages are they tied to and how many face to face visits should be included?
EAP services are not included in this RFP.
39. Ben Admin What BenAdmin platform is currently in place if any?
Refer to Question 5.
40. STD Is FICA included in STD Claims experience?
No.
