| Vendor Question | Response Summary | |
Regarding the Construction Addition and Renovations to Joint Ambulatory Care Clinic RFP, section 2.8 Proposal Requirements, B. Proposal Format:, 4. Volume III Administrative Information point h. requests the Offeror’s EMR and instructs offerors to fill out the attached document (Contractor Safety – Environmental Form). That form does not appear to be included in the provided attachments; can you please provide a copy of this form for offerors to complete? | See Attachment Contractor Safety – Environmental Form included in this amendment
Reference Solicitation document, A.3 Price/Cost Schedule: There is a UNIT PRICE line for each Item Number, each showing a quantity of (1), then also an AMOUNT line. It appears from the way each DESCRIPTION OF SUPPLIES/SERVICES is written, the intent is that UNIT PRICE line will be total cost specific to the stated scope for that line, but the AMOUNT line will be a summation of the current Item Number UNIT PRICE and the previous Item Number AMOUNT, in other words Item Number 0002 AMOUNT will be the “summation” of 0001 AMOUNT and the 0002 UNIT PRICE, then Item Number 0003 AMOUNT will be the “summation” of 0002 AMOUNT and the 0003 UNIT PRICE, and so on, such that Item Number 0005 AMOUNT will be Base Bid 0001 with all deduct Alternates taken into account. Please clarify if this is the intent of how pricing is to be submitted, and if so what should be shown in the GRAND TOTAL line. | The Grand Total Line was inadvertently in the solicitation. There is not a grand total. See Notice under A.3 Price/Cost Schedule
Specification 01 00 00 Paragraph 1.3.B contains Phasing information stating how each phase must be completed in its entirety and accepted by the VA prior to beginning work on the subsequent phase, then information in the paragraphs regarding Phases 2A and 2B state that the work for each can be performed concurrently with Phase 1. As Phases 2A and 2B are new parking areas completely apart from the Phase 1 Area F work, please confirm that work associated with Phases 2A and 2B can be performed concurrently with Phase 1. | Rationale of phasing confirmed to allow Contractor to perform Phase 2 as sub-phased in documents concurrently with Phase 1 build-out. The key to phasing is that Parking Phase 2A must be conducted independently from Phase 2B and is required to be the first parking lot expansion completed.
Sheet A-104 contains a Keynote 3 indicating a new mechanical penthouse, however this area appears to be shaded as an area where no work is to be performed, and we cannot find any other information in the documents regarding construction of this penthouse. Please confirm if there is any work on this project associated with this Keynote 3 penthouse. | New mechanical penthouse is not part of the scope of this project
Current Proposal Due date of February 5th is a Monday. As there are sufficient challenges and complexities associated with this project that make it difficult to discuss and finalize in the short timeframe of a Monday morning, please consider moving the bid date to at least Tuesday. | The proposal due date has been extended in this amendment
A.3 Price Cost Schedule indicates that Item Number 0002 removes appx. 51,200 SF of parking, and references sheet GI100 but does not specifically reference which parking area(s) are involved. GI100 Indicates Parking Areas A and C (as referenced on CS101) are together in Phase 2A, but GI100 also shows Parking Areas A and B as being together on Deductive Alternate D1. Appx square footages would seem to indicate that Parking Areas A and C should be together as Item Number 0002 and Parking Area B is Item Number 0003, please confirm. | CS101 Deductive Alternates are programmed correctly as is Phasing requirements. The issue is with GI100 as Deducts are mislabeled. Please, default to CS101 for understanding Deducts associated with Parking requirements. Phasing on both referenced plan sheets are correct.
Building Operational/Vacate 1st Floor? It is understood that the overall facility will/must remain fully operational during the course of this project. Is it the intent that the services in the First Floor area immediately below the Second Floor primary work area will remain operational? | It is accurate to state that no current VA operations located in the JACC (Pensacola Clinic) shall be closed and/or incur negative impacts to normal patient schedules and operations. The Physical Therapy and Prosthetics Clinics directly below the primary work area will be operational during all business hours.
Specification 01 45 00, Paragraph 3.4.D discusses Third Party personnel, and related Experience Matrix for the various disciplines. As all systems associated with this project are extensions to existing and not new to the facility, with multiple systems even being covered specifically per the Attachment 9 document, would it be allowable for this project to have these personnel be employed by subcontractors? | The roles must be employed by the GC.
Specification 01 00 00 states Contractor Personnel requirements in Part 1, Paragraph 1.2.E, and indicate that the Superintendent, SSHO, and CQC System Manager will all be separate individuals. 01 45 00 states in Paragraph 3.4.B that the CQC System Manager has duties as the project superintendent in addition to quality control. The Bidding Documents section of the Site Visit Presentation document seems to indicate that a “Division Breakdown Pricing Sheet” is part of the issued documents but we cannot find it. Please clarify if this Division Breakdown sheet will be provided or if the Offeror is to submit their own version. | Please omit any requirement for "division breakdown pricing sheet" from presentation. Only provide requirements required in soliciation documents Section 2.8 PROPOSAL REQUIREMENTS, Paragraph B.3.d (Page 11).

Please clarify if the project is Tax Exempt or not. | Special Notes regarding FAR 52.229-3: Federal, State and Local Taxes: Offerors are hereby notified that all state and local sales or use taxes for construction materials must be included in their bid or proposal price. It is solely the responsibility of the offeror to include all other applicable taxes in their bid or proposal price as well. The Government has determined that for this acquisition it cannot take advantage of any sales or use tax exemptions that may be available to it under applicable state law. Under no circumstances will any Contractor be designated as an agent of the Government for purposes of any tax exemption, nor will the Government authorize any Contractor’s use of any tax exemption directly applicable to the Government under this procurement. |
