1.0 BUILDING INFORMATION

Building #1 was originally constructed in 1928; a 1932 addition extended the building to the north and east, a 1938 addition extended the building to the south, a 1945 addition extended the building to the east and southeast, and a 2001 addition in-filled the south end of the building. Several smaller additions occurred in 1947, 1977, 1983, and 1992.

Building #9 was originally constructed in 1945; several small additions occurred in 1977, 1983, 1994, 2001, and 2002.

Building #46 was originally constructed in 1983; a 1997 addition extended the building to the northeast and a 1999 addition extended the building further to the northeast. A smaller addition occurred in 1990.

The interior floor finishes included carpet, floor tile, concrete, linoleum, vinyl flooring, wood, and epoxy flooring; the interior wall finishes included gypsum wallboard, plaster, wood, concrete, clay brick, brick, epoxy, ceramic tile, and concrete block ; and the interior ceiling finishes included ceiling tile, wood, concrete, plaster, gypsum wallboard, epoxy, and metal. The roofing system is a peaked asphalt shingled roof on the west end of building #9; and a flat rubber-membrane roof on the remainder of building #9 and on buildings #1 and #46; the exterior of the structure is brick.

The piping systems were insulated; fiberglass insulation (with hard fittings), aircell insulation (with hard fittings), millboard insulation (with hard fittings), and mag block insulation (with hard fittings) are located throughout the structure; collectively referred to as Asbestos Pipe Insulation. In addition, fiberglass insulation (without hard fittings) and neoprene insulation are located throughout the structure. Generator breeching and fireproofing were observed in the structure. HVAC systems located in the structure consisted of steam radiators and forced air furnaces equipped with heating/cooling coils.

2.0 ASBESTOS SURVEY INFORMATION

Buildings #1, #9, and #46 were surveyed as part of a larger project for the VA. This report is part of “Volume 1” of a two volume series. This report includes building specific information only;

please refer to the opening section of “Volume 1” for methodologies, definitions, and other pertinent supporting information.

A total of 380 samples were collected from suspect asbestos-containing materials (ACM) on July 28, 2007, an additional 13 samples were collected on September 16, 2008, and an additional 4 samples were collected on September 23, 2008. Laboratory analysis results indicate 51 of these samples tested positive for asbestos.

2.4 Survey Notes

LEGEND identified and sampled multiple types of Window Glazing in Buildings #1, #9, and #46;

the window glazing on the 1932, small 1945, and the large 1970s exterior windows contained asbestos. In addition, the window glazing on the reinforced window between rooms 3119 and 3120 contained asbestos.

LEGEND identified and sampled multiple types of Window Caulk in Buildings #1, #9, and #46; the window glazing on the 1928 and 1932 exterior windows contained asbestos.

LEGEND identified and sampled multiple types of Joint Compound in Buildings #1, #9, and #46;

the joint compound from the 1966 renovations between rooms 1086d and 1086e and also between rooms 3140, 3141, and the adjacent hallway contained asbestos.

LEGEND No. 0800912 BUILDINGS #1, #9, and #46 Page 4 of 4

LEGEND indentified and sampled multiple types of Sink Undercoating and Sink/Counter System Undercoating in Buildings #1, #9, and #46; the black sink undercoating on the sinks in rooms 2166 and 3160 and the black sink/counter system undercoating on the autopsy sink/counters in the center of B79e contained asbestos.

LEGEND identified a Transite Pipe running vertically through room 3048; LEGEND was unable to locate the pipe continuing in either the 2nd or 4th floors. When destructive surveys are feasible in rooms 2043 and 4012; further inspections should be conducted to verify the possible presence of the transite pipe.

LEGEND indentified Transite Panels (behind registers) throughout the 1945 areas of Building #1 and #9; however, several areas existed where access to the exterior walls has been blocked (for example room 2153 where a cooler is now installed). It is possible that transite panels (behind register) may remain in these areas; if found during future renovations, they must be treated as asbestos containing materials.

LEGEND observed multiple areas in Buildings #1 and #9 where multiple layers of flooring may exist; several wings of Buildings #1 and #9 were recently renovated and were not inspected for multiple layers of flooring (wings 2B, 3A, 3B, parts of 4A, and the north end of Building #1 3rd floor). The Multiple Flooring Layers Locations Diagrams included in the ACM Locations Diagrams section identify areas where indications of multiple layers of flooring existed; the diagrams are representative of the current building conditions at the time of the survey. Notably, Building #46 does not contain ACM flooring in either single layer or multiple layer flooring areas. The follow recommendations correlate to the Multiple Flooring Layers Locations Diagrams:

A. Adjacent single layer flooring materials are negative; LEGEND recommends a destructive survey of this room be completed and the lower layers of flooring be sampled and analyzed to determine asbestos content.

B. Adjacent single layer flooring materials are positive; LEGEND recommends abating all layers of flooring in this room.

C. Observations made by LEGEND during the survey indicate the lower levels of flooring in this room to be asbestos containing; LEGEND recommends abating all layers of flooring in this room.

D. This area likely contains a different type of lower level flooring as compared to the rest of the room; LEGEND recommends a destructive survey of this area be completed independent of the rest of the room and the lower layers of flooring be sampled and analyzed to determine asbestos content.

E. Throughout the north end of Building #9 1st floor LEGEND observed numerous indications of single layer of flooring and numerous indications of multiple layers of flooring; LEGEND recommends a destructive survey of this entire area be completed and the lower layers of flooring (if present) be sampled and analyzed to determine asbestos content.

F. In this area LEGEND observed numerous indications of single layer of flooring and numerous indications of multiple layers of flooring; LEGEND recommends a destructive survey of this entire area be completed and the lower layers of flooring (if present) be sampled and analyzed to determine asbestos content.
