1. When would you like SF1442 and reps & certs to be sent? Phase I or Phase II?

*Reference I.5B - PROPOSAL REQUIREMENTS and 1.7B - REQUIREMENTS

FOR ELECTRONIC SUBMISSION OF OFFER.

2. Previous SLC VA projects have allowed submission of relevant projects which are more than 80% complete.

Will the VA allow past performance for recent design build projects as long as they are

80% complete at an active medical center?

(The following is reference from a previous SLC VA solicitation) For purposes of this evaluation, a relevant project is further defined as a construction of an addition to a medical facility. Project(s) shall have a minimum value of $$ and completed, including all options, within the past five years from date of issuance of this RPF, or ongoing projects more than 80% complete including all options.

No – projects need to be 100% completed.

3. Will design require update for VA Barrier Free Design Standard (Rev 11/1/18)

Yes.

4. Please clarify what portions of the project will be included in the “20% of existing layout to need updating”. For example, will it include any of the façade or structural footprint?

Or will it be limited to redesign of interior space only?

Interior space and potential structural update to support future patient lift system.

Note patient lifts are not to be purchased and installed with construction dollars, however design will need to account for and plan for patient lifts.

5. Is the extent, size and location of the existing storm drain system identified and can a detailed as-built plan be provided to the A/E team?

VA has as-built plan of existing storm drain system available for A/E team.

6. Has the VA adopted any new design standards regarding infection control for waiting areas or Emergency Departments since 2016? If so can those be shared with the A/E team?

Support Expansion / 660-800 – v2:

No, VA has not adopted new design standards regarding infection control for waiting areas/ED.

7. The original Urgent Care Expansion Project design included a structural system engineered for a future partial 2nd level expansion. Has anything changed regarding that scope?

No change.

8. Please clarify in detail what is expected by the statement: … “reviewing/refreshing a previously submitted complete design…”

Design should be updated to comply with current codes. Some interior future in architectural layout is not to scale and will need adjusting.

9. Is there an Architect’s version of the Attachment 13 Past Performance Questionnaire, or shall they utilize the Contractor’s version?

I believe they utilize the contractor version, not aware of a different PPQ version for

A/E.

10. Please confirm that Factor 2, Technical Approach (page 13 of 68) is truly only one page?

This seems rather short for the requested content.

One page is the limit.

11. Attached is the layout of the existing hospital, clearly showing that what we are renovating, and expanding is not the current emergency department. Google shots from the adjacent street confirm this. No current entrances from the street. So I believe the construction process will be much simpler than we originally thought.

It would be nice if we can figure out what department it is that we’re working on, just to show knowledge in our write up. (See attached Hospital Layout)

Area of work is highlighted in attachment. The “V” portion of the building 1 in the area of work has already been demolished in a separate project in preparation for this one. Note the box drawn on layout is not exact size, but it does give location of where the ED will be attached to building 1. The space for renovation for located in existing building 1 directly adjacent to planned addition. Please refer to drawings for exact locations, planned traffic flows, and spaces inside for renovation details.
