Renovate Surgical Oncology, Building 31, First Floor 2 May 2019 Loma Linda VA Medical Center 100% Design Development Submittal
BASIS OF DESIGN
A. GENERAL
This design narrative provides a description of this Renovation project for Surgical Oncology, Building 31, at the Jerry L. Pettis Memorial Veterans Medical Center, located in Loma Linda, California. Building 31 is an 18,309-square-foot, two-story, steel-frame building that was constructed in 2014. The first floor of the building was not completed as part of the original construction due to budget constraints. At this time, the Veterans Affairs Loma Linda (VA-LL) would like to complete the construction of the first floor and also make modifications to the Pharmacy located on the second floor, as required to bring the pharmacy into compliance with USP 800, the United States Pharmacopeial Convention's standards on handling hazardous drugs in healthcare settings. The VA must comply with a directive that mandates the pharmacy in Building 31 be in compliance with USP 800 by July 1, 2018.
As such, this project will renovate 8,000 GSF of non-clinical space on the first floor of Building 31. The interiors of the space are primarily left unfinished, while the shell was completed as part of the original construction. Approximately 6,000 GSF of this space will be renovated into clinical exam space for Surgical Oncology, and a combined waiting and patient-support area for the building will be provided. The project will also reroute the critical power and provide redundant IT connectivity for the building. A screen wall for the mechanical equipment at the top of the building to gain efficiencies in system operations and improve aesthetics will be provided.
Site improvements must be made to bring Building 31 into compliance with the new VA Physical Security requirements. The existing parking stalls within 25 feet of the building face will be eliminated in order to provide the 25-foot standoff distance required by the Physical Security Design Manual. New bollards will be provided and other site improvements, such as landscaping and wayfinding, will be made to maintain accessibility requirements with the new standoff.
Please refer to the discipline-specific sections that follow for more detail on the project requirements.

F. STRUCTURAL
1. Scope of Work
a. Project Description
The first-floor interior of Building 31 is largely unfinished; however, the structure is mostly complete. The central corridor area of the first floor, which includes the corridor, elevator, elevator machine room, electrical room, waiting and reception area, an office, and the stairs, is the only area that is completely finished from a structural perspective. Within Areas A and B, as indicated in the drawings, the existing structure has been completely installed and finished with the exception of the floor slab. It appears that a 1-inch-thick unreinforced concrete slab was poured in this location in order to protect the supporting soils beneath; however, the tops of the grade beams and square pile caps in this area remain exposed.
Existing #4 rebar dowels remain in place to tie the existing grade beams, square pile caps, and finished slab within the central corridor area into the new concrete slab when finished. The second floor and roof of Building 31 consist of a completed structure.
This project will complete the installation of the reinforced concrete slab on the first floor of Building 31 within Areas A and B that fall outside of the finished central corridor. The existing dowels at the grade beams and pile caps will be unbundled and bent back into place as required to tie in to the new slab. The existing 1-inch topping that is currently covering the soil base within the unfinished Areas A and B will be removed in its entirety. The upper 6 inches of the soil base, or deeper if disturbed more than 6 inches, will need to be recompacted for placement of the new concrete slab. The new slab is expected to be a 5-inch concrete slab with #4 reinforcement at 18 inches on center each way, matching the existing slab. The top of the new slab will match the elevation of the top of the existing central corridor slab.

There is currently no anticipated structural work required on the second floor due to the renovation of the pharmacy area.
This project will also include providing mechanical screens for the large pieces of mechanical equipment mounted on the roof. These screens will be constructed in a very similar manner with respect to the original mechanical screening that was not constructed. The mechanical screen will be attached to structural steel framing consisting of HSS8x4 top and middle rail support members and HSS8x8 posts located at locations similar to the original design locations. Through field investigation, it was verified that certain wide-flange steel framing members that were intended to support the mechanical screening in the original building design were not installed. As such, this project will install these supporting members underneath the roof structure and attached to the existing steel structure.

a. Project Description
The Loma Linda VA Surgical Oncology, Building 31, is being renovated.
First Floor: This floor is largely undeveloped on the west and east sides of the building at the start of renovation. Major ductwork was installed in order to provide main duct branches for systems that were installed later. The new design renovation has designed the first floor as a typical office and exam layout. New ductwork, piping, and constant air volume (CAV) or variable air volume (VAV) boxes will need to be installed to serve these spaces.
Second Floor: It is anticipated that the only floor plan changes to the second floor will be an approximately 70 SF expansion of the existing 540 SF pharmacy. The expansion will be into an existing open area/hallway. It will need to be determined in a later phase if the air handling unit (AHU) that is dedicated to the pharmacy will need to be modified or replaced to accommodate this expansion.
Existing ductwork may need to be modified, and new ductwork may need to be added for the pharmacy expansion. Two fume hoods serve the pharmacy and are currently next to each other and share the same exhaust. In the proposed plan, one of the fume hoods will be moved and require a dedicated exhaust fan, which will require a new opening in the roof.

Common areas (corridors, reception, waiting, lounge) will contain 6-inch downlights, lay-in fixtures, and decorative pendant fixtures, depending on the ceiling type. Fixtures located in the center of corridors will have shielding for direct/indirect distribution.
Patient care areas and exam rooms will contain downlights and 2'x2' or 2'x4' LED fixtures. Fixtures located directly over the patient exam areas will be shielded with asymmetric distribution such that the LED source is not directly visible from below.
Restroom lighting will consist of wall mounted vanity LED fixtures and LED downlights.
Office, Workrooms, and Administrative areas will have 2'x2' or 2'x4' LED fixtures.
Utility and Storage room lighting will consist of lensed LED strip fixtures, either surface or flush mounted. Mechanical and electrical rooms will have surface or pendant mounted LED strip fixtures.
Egress lighting will be circuited to an Emergency life safety branch power circuit. Some selected lighting will be connected to the critical branch. The remaining lighting will be connected to Normal power circuits.
Lighting controls will comply with California Title 24 requirements. Areas lit by skylights and windows will have automatic dimming sensors to lower light fixture illumination levels during day light hours. Individual rooms (exam, office, workroom, restroom, storage, utility) will require manual activation to turn on fixtures. Vacancy sensors in these areas will turn off fixtures after a set period of time (adjustable).
I. TELECOMMUNICATIONS
Telephone and data network receptacles will be installed where indicated by the telecommunications designer. A 4-inch-square box with a single-gang mud ring will be roughed-in at each telecommunications receptacle location. A 1-inch (minimum) EMT conduit with bushings and a pull-string will be stubbed from the receptacle backbox to an accessible location above the ceiling.
A cable tray system will be installed along the edge of the corridors above the ceiling to manage the majority of the telecommunication cable routing from the IT closets to the telecommunication receptacles. Additional cable management will use J-hooks mounted above the ceiling to support cable. Conduit sleeves will be installed for pathways that penetrate floor levels and fire walls. Fire Rated cable transition assemblies will be installed as required through fire rated walls and floors.
Separate cable pathways will be provided as needed for the following systems:
• Security
• Public Address/Intercom
• Nurse Call
• Cable TV

J. PHYSICAL SECURITY
The VA-LL has identified Building 31 as a Life-Safety Protected Facility, and, as such, Building 31 will be brought into compliance with the site considerations of the Physical Security Design Manual for Life-Safety Protected Facilities, dated January 2015. The Physical Security Design Manual defines a Life-Safety Protected Facility as a facility that is required to protect the life safety of patients, staff, and visitors in case of an emergency; although indispensable to the mission of the VA, they are not required to remain operational in a natural or manmade extreme event or a national emergency. Refer to each discipline’s design narrative for the specific details on compliance measures for physical security.
