The Department of the Veterans Affairs, NCO-15, intends to award a Construction Contract for Project 589A7-21-700, EHRM Infrastructure Upgrades at the Robert J. Dole VA Medical Center, 5500 E. Kellogg Ave., Wichita, Kansas. 67218 The NAICS Code for this project is 236220 - Commercial and Institutional Building Construction. The size standard is $39.5 million. Estimate of construction magnitude for this project is $10 million to $20 million.

This project is being solicited as a Service Disabled Veteran Owned Small Business (SDVOSB) set aside.

Proposals received in response to this solicitation will be evaluated in accordance with the procedures outlined in the Federal Acquisition Regulation FAR 15.101-1, Best Value - Tradeoff source selection process.

Offerors not providing the documentation outlined in the "Instructions to Offerors" will be rated unacceptable and will be ineligible for award. Physical copies of the proposal shall not be accepted.

There will be only one site visit for this project on 01-14-2022 at 11:30 A.M Central Time in the Bldg. 10 conference room at the Wichita VA Medical Center, 5500 E. Kellogg, Wichita, Kansas 67218.

Construction Scope of Work Electronic Health Records Management (EHRM) Infrastructure Upgrades

SCOPE OF WORK (SOW):

Contractor shall completely prepare project site for operations, including demolition and removal of existing site elements, and furnish labor and materials and perform work for the VA Wichita Health Care System Project 589A7-21-700, Electronic Health Records Management (EHRM) Infrastructure Upgrades as required by drawings and specifications. Period of performance is projected to be four-hundred twenty-five (425) calendar days from the Notice to Proceed.

To prepare the VA campus for EHRM Upgrades, main campus of Robert J. Dole VA Medical Campus will require a near-complete re-cabling of the main hospital and associated buildings. This project will include all materials and labor to replace all telecommunications cable, outlets, and associated equipment for buildings in this project at the Robert J. Dole VA Medical Campus. Multiple buildings on campus include Buildings B1, B1A, B1B, B1C, B2, B3, B4, B5, B5B, B6, B7, B11, B12, B13, B16, B19, B20, B21, B26, B29, B34, B59, B60, B61, and B62. The scope also includes replacement of all fiber optic cabling, conduit and providing new conduit, fiber and manholes serving the buildings' Telecommunications Rooms (TR) in accordance with the drawings and specifications. Project includes replacing the existing multi-mode fiber service to each building with new OM4-10GB fiber. New fiber shall be separate homeruns to Building Demarcation room from each building and existing daisy chains will be removed. For all new conduit routes under this contract, a redundant, spare conduit shall be installed parallel with each new route. All OM4-10GB fiber from exterior buildings will run to fiber demarcation room in Building 34 before bridging up to corresponding fiber cans in the B19 server room.

OM4 fiber will be split between cores. Campus Area Network (CAN) fiber connections will be unbroken (non-bridged/spliced) homeruns back to the main fiber distribution panel (FDP). Fiber connections at building should not be run through additional buildings unless pass-through or mesh; no “daisy chaining” of fiber will be allowed and all fiber optic cable will terminate in LC connectors.

For the Internet Service Provider (ISP) fiber WAN connection, contractor shall install new conduit and manholes as shown to route redundant ISP WAN fiber to the new demarcation room in B34. ISP shall pull fiber for this WAN connection; contractor shall provide the infrastructure (conduit and pull boxes/manholes). Contractor shall coordinate with ISP for conduit requirements and access requirements.

New site fiber pedestal to occur a minimum of 20’-0” from existing provider entrance to meet redundant route requirements and shall maintain minimum 20’-0” separation on the route to new location. A minimum of two (2) 4” conduit will be required for this entire route. Specific conduit to be as required by service provider.

Un-terminated "future capacity" cables are not permitted. All installed cables shall be terminated at each end and documentation, labeling, and (where applicable) test results provided. This applies to all permanently installed cable types. Building fiber will be smoke-rated OM4-10GB. In addition to replacing all of the communications cabling, an additional 15% capacity shall be provided as shown on the drawings and specifications. All Copper replacement will be CAT 6A.

Any conduit used will be marked appropriately with “low-voltage/data” at regular intervals to be visible in interstitial spaces with minimal effort. Existing conduit drops in-wall shall be retained and reused;

contractor shall not exceed conduit fill percentage code requirements with the new cable drops. CAT6A cabling installed for end user devices (terminated at wall plates) will follow the red/blue/yellow standards as outlined in Specification 27 15 00. The facility has a required 1 Gbe connectivity to desktops, and other EUD, and this will include the replacement of wall jacks at EUDs to become CAT6A compliant. All in-building copper cabling will be terminated and labeled in accordance with the drawings and specifications and will be terminated to CAT6A modular jacks capable of accepting and functioning with other modular (i.e.: RJ11) plugs without damage to the jack. CAT 6A color coding shall be provided for all patch cables and all primary cable to each EUD, as follows:

For every TR in the SOW, Contractor shall provide and install new patch panels conforming to EIA/ECA 310-3 dimensions and suitable for mounting in standard equipment racks, with (48) RJ45 jacks aligned in two horizontal rows per patch panel and labeled in accordance with the drawings and specifications.

Contractor shall also supply and label all new CAT6A patch cables for every device in the telecom room, at the end user device, and at regular 4’ intervals along the entire cable length in accordance with 27 15 00.

Each TR shall also be provided with new four-post racks with minimum 40% spare capacity for future expansion. All racks and patch panels will be labeled in accordance with the Drawings and Specification 27 15 00. Contractor shall provide and install and utilize a series of welded cabling support structures (“ladder racks”) around the perimeter of the telecom room to support the weight of the cable. No wireless access points will be replaced and will be protected-in-place, only the cabling will be replaced. All cabling for wireless access points will be labeled in accordance with the specifications and shall be purple in color. This replacement process will require a temporarily located rack adjacent to the existing rack.

Contractor shall replace existing circuits one-for-one and locate on new patch panels on the new rack and provide interim longer-length patch cables to existing devices/switches. Existing equipment to remain will then be relocated to the new rack and the new rack shall be mounted in its final permanent location.

All existing analog phone cable devices and terminations etc. shall remain operational at all times and shall be retained and reused. Protect in place in existing location.

The project scope also includes addition of new Computer Room Air-Conditioning split systems with temperature and humidity monitoring station for all locations, as well as the addition of new dedicated electrical panels, transformers, and circuits.

Project scope also includes providing new PACS devices, card readers, control panels, power supplies and associated electrical and low voltage service.

All construction is to be performed in one phase during normal working hours as much as feasible. Off-hours premium time is included for noise abatement and utility shutdowns only. Shutdowns shall be limited to 8 hours maximum and scheduled for minimal interference with hospital operations. Any item(s) to be salvaged by the VA are to be removed by the VA from the site before construction begins.

Contractor to provide dumpsters for disposal of construction debris and materials. The construction contractor will be required to submit construction waste debris reports to include weight of construction waste, weight of any recycled materials and the cost for removing construction waste. The construction contractor shall comply with VA and Medical Center guidance to ensure infection controls during the construction.

1. Normal Working Hours are 0700 to 1600, Monday to Friday.

2. Working outside these hours requires prior written approval from the COR. Due to the work site location, it is anticipated that some off-hours work will be required to perform this project work to manage noise and vibration impacts to Veterans Patients and Medical Staff performing care.

Contractor shall execute electrical, communications and security systems installations in accordance with TIA, VA and BICSI installation standards and the project specifications and drawings. The Contractor shall provide copies of system manufacturer certification for all technicians prior to commencing any work at the Wichita VA Medical Center. The Contractor shall provide proof project superintendent with BICSI Certified Commercial Installer Level 1, Level 2, or Technician to provide oversight of the project.

Cable installer must have on staff a Registered Communication Distribution Designer (RCDD) certified by Building Industry Consulting Service International (BICSI). The RCDD licensed staff member shall be on-site throughout the life of the project to oversee all telecommunications and security scope and shall provide consistent oversight of the project cabling throughout design, layout, installation, termination, and testing. Work includes general construction, alterations, and certain other items. Contractor shall provide services to include but not limited to furnishing all supervision, labor, equipment, materials and transportation necessary to accomplish work for the demolition and replacement as required by specifications and drawings.

Comprehensive independent 3rd party inspections, testing and written reports for all Divisions are required and paid by the General Contractor. Contractor shall employ independent third party inspection consultants to perform all inspections required by this contract. Minimum qualifications for each Division’s inspector are identified in 01 45 00 “Experience Matrix”. Third-Party inspections are in addition to, and independent of, the General Contractor’s primary Quality Control Manager and associated protocol or Government/AE Construction Period Services

Contractor shall appoint an independent third-party individual, company or firm to act as the Commissioning Agent. The Commissioning Agent is the designated person, company, or entity that plans, schedules, and coordinates the commissioning team to implement the commissioning process.

Contractor is responsible for Asbestos Containing Materials (ACM) removal and mitigation where it occurs inside area of work. There will be no cost to the Government for any removal, testing, oversight or clearance of ACM. Traditional and Glovebag Abatement methods as stipulated in Specification Sections 02 82 11 and 02 82 13.13, are the only allowable methods of abatement. No alternate abatement methods will be considered.

Electrical, Telecommunications, and Security Work: Contractor shall provide all labor, material, equipment and supervision to perform the required electrical and telecommunications construction work on this project including materials and construction equipment to provide new telecommunications Cat 6A cable throughout, new outlets, and new racks, patch panels, UPS, fiber panels and associated equipment for all buildings. Project also includes the complete cabling of all site multimode and single mode fiber distribution to each building. Conduit and manholes shall also be provided for redundant ISP WAN connections (ISP to provide fiber and terminations).

Provide materials to install and wire four additional 120 volt 20 amp duplex receptacles and two (2) new 208V 3P NEMA 30 amp twist lock receptacles to serve new equipment racks. Provide new electrical panels, transformers, circuit breakers and distribution to serve the new receptacles. Contractor shall perform 30-day load monitoring prior to providing the new circuit to confirm available capacity. All the new circuits shall be wired to the emergency power sources indicated.

For communications functions, provide cable trays, equipment racks with vertical/horizontal wire management. Replace all fiber optic lines serving the TRs. Provide network drops in each new closet.

Provide a new digital, wall mounted CCTV (Closed Circuit Television) camera in each closet, camera shall comply with Campus standards (Basis of Design Wisenet XNP-6320: IP addressable network camera, POE+, PTZ, motion sensor, auto-tracking, dome with wall mount. Integrate camera into existing VMS. Provide controllability through existing IT network).

Provide Physical Security Access Control (PACS) for all the TR doors (including controllers and power supplies), coordinate with the door hardware specification for additional requirements.

For all TRs in all buildings, provide a dedicated bonding system. The bonding systems will include a secondary bonding bar in each closet and the bonding of equipment located in each closet to the new bonding bar.

Contractor shall provide a 30-day load test on panels indicated on the one-line for each Building. This test is required by NEC 220.87 for verification that no loads have been added since the design level survey.

Contractor shall provide load monitoring data to the COR/AE. New electrical panels and electrical service branch circuits are specified by the scope of work and shall be furnished by the contractor. Load monitoring shall also be performed on all panels upstream of the new branch circuit panels.

Mechanical, Plumbing and Fire Protection Work: Work includes all labor, material, equipment, and supervision to perform the required Mechanical, Plumbing and Fire Protection construction work on this project including new dedicated split system Computer Room Air Conditioner (CRAC), condensate pumps and drains, reconfiguring and certifying existing wet pipe fire protection system, and providing dedicated clean-agent fire suppression systems for Buildings without existing wet-pipe service. New CRAC Work includes: (1) installation of the indoor unit above the cable tray at the entrance of the closet;

(2) installation of the associated condensing unit on the building roof or site with housekeeping pads; (3) installation of a new thermostat in each IT closet; (4) installation of an integrated condensate pump, trap and drain line; and (5) installation/routing refrigerant and control lines between the indoor and outdoor units, providing refrigerant piping supports, roof penetration, curb and hood and wall penetrations.

Refrigerant routing, structural supports, and penetrations shall be field verified and coordinated with the VA. Contractor shall submit coordination drawings for approval showing the proposed routing, supports and penetrations prior to beginning any work. Provide space temperature sensor, space humidity sensor, auxiliary drain pan moisture sensor (not float switch) and supply air temperature sensors on all CRAC units connect these to the central Engineering Control Center (ECC) system for trending and alarm purposes via BACNet with the manufacturer provided BACnet converter. All new split systems shall be addressable and controllable via the existing ECC. The central ECC system shall monitor space conditions and unit status and shall alert the boiler plant operator and HVAC shop whenever temperature or humidity are out of tolerance; when water is in the HVAC secondary drain pan; and when any supply air temperature does not match what is required by room conditions. Contractor shall submit coordination drawings prior to commencing work showing new and existing systems and all provisions for coordinating existing utilities.

Provide Fire/Smoke damper for each TR to match wall rating. Dampers shall be installed on existing duct serving existing diffusers and return grilles. Coordinate with existing conditions and submit coordination drawings in accordance with 23 05 11. Assume (2) 10” round dampers per TR, power supply, and controls.

For fire suppression, contractor shall reconfigure and/or provide additional sprinkler heads to the existing building automatic sprinkler system as shown on the drawings. TR spaces in Buildings 6, 7, 13, and 21 shall be provided with dedicated clean-agent fire suppression systems as shown on the drawings.

Contractor shall coordinate sprinkler coverage requirements with all temporary ICRA barriers to ensure all areas are properly protected during construction. For all telecommunications rooms (TR), the contractor is responsible for providing a new fire extinguisher cabinet with a Type 2A:10B:C fire extinguisher located no greater than 75 feet away from the new TR. Contractor to coordinate the new location with the COR. Contractor shall bid a new FEC for each TR in the Scope of Work. See specification 10 44 13.

The existing fire alarm system will be modified by relocating and/or adding compatible smoke detectors to the existing building fire alarm system.

All new and existing systems within the Scope of Work shall be commissioned (or recommissioned) for each building per Specification 01 91 00.

The expected construction period duration for each building includes all pre-construction activity and post-construction testing, third-party inspections, and commissioning. Contractor shall submit a comprehensive schedule of work in compliance with 01 32 16.15 PROJECT SCHEDULES (SMALL PROJECTS DESIGN/BID/BUILD). Contractor shall submit and obtain approval for all project submittals prior to beginning physical construction. Contractor to perform Commissioning of all new systems and existing systems impacted by the project according to requirements in sections: 01 91 00 – GENERAL

COMMISSIONING REQUIREMENTS; 23 08 00 – COMMISSIONING OF HVAC SYSTEMS; 26 08

00 – COMMISSIONING OF ELECTRICAL SYSTEMS; 27 08 00 – COMMISSIONING OF

COMMUNICATIONS SYSTEMS and 28 08 00 – COMMISSIONING OF ELECTRONIC SAFETY AND SECURITY SYSTEMS. A detailed commissioning plan is required to ensure that all devices are calibrated, programmed, and all graphic displays are accurate and approved by applicable VA staff. Each Building shall be commissioned and accepted by the VA prior to commissioning the next building.

1. PREPARATION AND SUBMITTAL OF PROPOSALS

a. GENERAL INFORMATION.

Selection of an offeror for contract award will be made on the basis of an assessment of each offeror’s response to the Request for Proposal (RFP). All proposals shall be subject to evaluation by a team of government personnel. The government reserves the right to award without discussions based upon the initial evaluation of the proposals. The proposals will be evaluated in accordance with the content provided in the solicitation. Using the Tradeoff process under the Best Value, in accordance with Federal Acquisition Regulation (FAR) Part 15.101-1, it may be in the best interest of the Government to consider award to other than the lowest priced offeror or other than the highest technically rated offeror. All evaluation factors other than price, when combined are approximately equal to price. Evaluations will be based on the following evaluation factors:

These instructions prescribe the format for the proposal and describe the approach for the development and presentation of proposal data. These instructions are designed to ensure the submission of necessary information to provide for the understanding and comprehensive evaluation of proposals.

c. AWARD BASED ON INITIAL OFFERS.

Pursuant to clause 52.215-1 (included by reference), the Government intends to evaluate proposals and award a contract without discussions with offerors (except clarifications as described in FAR 15.306(a)). Therefore, the offeror’s initial proposal should contain the offeror’s best terms from a cost or price, and technical qualifications standpoint. The VA reserves the right to conduct discussions if the Contracting Officer later determines such discussions to be necessary.

b. OFFER GUARANTEE (BID BOND). In accordance with solicitation provision 52.228-1, Bid Guarantee, failure to furnish a bid guarantee, in the proper form and amount, by the deadline established for submitting offers, may be cause for rejection of the offer. Bid guarantee shall be 20 % percent of the bid price or $3,000,000 whichever is less.

c. DOCUMENTATION OF TECHNICAL CAPABILITY. Proposals received in response to this solicitation will be evaluated in accordance with procedures outlined in Federal Acquisition Regulations (FAR) Part 15.101-1 for the Best Value – Tradeoff source selection process.

The following NON-Price Evaluation Factors will be used as the basis for determining Technical Capability: Relevant Experience of the Prime Contractor, Project Execution, Technical Qualifications of Proposed Staffing, Past Performance, and Prime Contractor’s Safety Plan.

NOTE: There is a realistic possibility that some offerors may choose to submit the same construction project as a qualifying example for both the past performance and the construction experience evaluation factors. This is not prohibited, but the supporting documentation must demonstrate that the submitted projects absolutely do comply with the specific minimal requirements, identified below, for each separate evaluation factor.

FACTOR 1: RELEVANT EXPERIENCE OF THE PRIME CONTRACTOR.

To be evaluated under this factor, Offerors shall submit a minimum of two (2) and maximum of five (5) construction project examples for the Prime Contractor that best demonstrates recent and relevant experience by the Prime Contractor on projects that are similar in size, scope, and complexity to the RFP requirements. Subcontractor work cannot be substituted and shall not be accepted. The projects must demonstrate an Offeror’s construction experience as a prime construction contractor directly responsible to the project owner and managing multiple subcontractors.

The offeror shall provide a short description of the project and state what work fulfilled the relevant experience areas for Infrastructure Upgrades including but not limited to: electrical, communications, HVAC, fiber replacement and/or installation, patch panel upgrades, data drops, data closets, upgrading cables to CAT 6A, providing a new fiber backbone, HVAC cooling and controls, and emergency power upgrades.

Project(s) should have a minimum value of $10,000,000.00 and be 100% complete. Projects submitted by the Offeror shall be completed within the past five (5) years from the date of issuance of this RFP. If the contract/work is incomplete or was completed more than five years prior to the date of submitting this proposal, it will not be deemed relevant and shall not be considered. Projects which are similar in size, scope, and complexity and were at an active Hospital, Medical Center, or project site with 24 hour a day operations are preferred and will be rated higher.

A project is defined as a construction project performed under a single task order or contract.

For multiple award and indefinite delivery/indefinite quantity type contracts, the contract as a whole shall not be submitted as a project; rather Offerors shall submit the work performed under a task order as a project.

Offeror’s submission should include at a minimum:

Provide the Project Title and Description, Project Location (Physical Address); Contract Type (Example: Design Build, Design Bid Build).

Provide the project owner name and telephone number of the owner’s contact person.

Provide a description of the scope of work, to include the work performed by the prime contractor. Scope of work includes information on subcontractors managed during the project.

Provide project statistics including start and completion dates (original and actual) and project cost (original and actual). If original and actual completion dates and project cost differ, provide an explanation as to why they differ.

If the Offeror is a Joint Venture (JV), relevant project experience should be submitted for projects completed by the Joint Venture entity. If the Joint Venture does not have shared experience, projects shall be submitted for each Joint Venture partner. Offerors who fail to submit experience for all Joint Venture partners may be rated lower.

(Note: If submitting as a Prime/Sub teaming arrangement under FAR 9.601(2), the designated prime must meet the specific requirements of Factor 1 “Relevant Experience of the Prime Contractor” in order for the teaming arrangement to be deemed acceptable.

Subcontractor experience will not be considered.)

FACTOR 2: PROJECT EXECUTION

To be evaluated under this factor, offerors must:

Prepare and submit a practicable construction schedule showing all necessary work elements to complete the project identified under this solicitation within the time frame specified on the SF1442. The schedule must be in the form of a progress chart of suitable scale to indicate appropriately the percentage of work scheduled for completion by any given date during the performance period. The submitted schedule must be developed using project scheduling software such as MS Project, Primavera, or any other comparable form.

Each Offeror's construction schedule must address the following:

Order of work elements to include project phasing, Submittals, Mobilization, Demolition, Construction, Equipment Deliveries and Lead Time, Installation of new Equipment, Punch List, De-Mobilization.

Number of days for each work element

Identification of long lead time materials

Identification of separate work elements

The VA’s review and approval of RFI’s, Submittals and other pertinent documents.

Interruption of VA Facilities utilities, delivery of Government furnished equipment, rough-in drawings, project phasing and any other specification requirements.

The proposed schedule must include the activities which are consistent with those described within the project specifications and drawings and must not exceed the Government's maximum allowable performance period, as noted in Block 11 of the SF 1442. The project will start at the issuance of the Notice to Proceed. For scheduling purposes, assume the Notice to Proceed will be issued thirty (30) calendar days after the award date. (Note that there is no guarantee the Notice to Proceed will be issued within this timeframe.)

*The schedule submitted with the offer should not be construed as the agreed upon schedule as per FAR 52.236-15. The awarded contractor shall, within five (5) days after receiving the Notice to Proceed, submit a revised schedule which will supersede the proposed schedule. The revised schedule will be subject to VA approval and must show the project being accomplished within the time frame specified on the SF 1442.

Provide Narrative for Project Challenges:

Describe in a written narrative, the challenges identified for this project and a plan for addressing these challenges. This plan shall include contractor’s plan to minimize the disruption of patient care to the greatest extent possible.

FACTOR 3: TECHNICAL QUALIFICATIONS OF PROPOSED STAFFING

To be evaluated under this factor, qualified offerors must submit a detailed resume of each key individual on the Prime contractor’s staff for his project. To promote maximum continuity on this project, key staffing positions are not to be subcontracted out. This factor will be evaluated to determine if contractor’s key personnel have the appropriate technical training and or previous construction experience to meet the technical requirements of the key staffing positions for a project of this nature. No one individual can perform more than two of the key staffing positions below:

(a) On-Site Superintendent – Must have a minimum of 5 years of experience as a site superintendent and must have OSHA 30 training. Provide references (that can be contacted) for a minimum of 2 projects with similar scope to this project (include brief project description).

To be considered for evaluation, the 2 projects must have been performed within the last five years and valid contact information must be provided.

If Reference cannot be verified, then the proposal will receive no further consideration

(b) Quality Control Manager – Must have a minimum of 5 years of experience as a project manager. Provide references (that can be contacted) for a minimum of 2 projects with similar scope to this project (include brief project description). To be considered for evaluation, the 2 projects must have been performed within the last five years and valid contact information must be provided. If Reference cannot be verified, then the proposal will receive no further consideration.

(c) Safety Officer – Must have a minimum of 5 years of experience as a safety Officer Must provide an OSHA 30 Certificate and a list of safety training that at a minimum includes “life safety, excavation safety, scaffold safety, fall protection training, crane/rigging safety”. Provide references (that can be contacted) for a minimum of 2 projects with similar scope to this project (include brief project description). To be considered for evaluation the 2 projects must have been performed within the last five years and valid contact information must be provided.

If Reference cannot be verified, then the proposal will receive no further consideration.

(d) The telecommunications contractor shall be a BICSI licensed Contractor with a minimum of five (5) years’ experience installing and servicing systems of similar scope and complexity. The Contractor shall provide four (4) current references from clients with systems of similar scope and complexity which became operational in the past three (3) years. At least three (3) of the references shall be utilizing the same system components, in a similar configuration as the proposed system. The references must include a current point of contact, company or agency name, address, telephone number, complete system description, date of completion, and approximate cost of the project. If reference cannot be verified, the proposal will receive no further consideration.

Special Note: Only submit information for personnel and subcontractors that will perform on a resulting contract. Any substitutions after award must be determined equally or more qualified by the Contracting Officer.

Special Note: During the period of performance of the Contract, the Contractor shall make NO substitutions of the Staffing unless the substitution is necessitated by illness, death, or termination of employment. The contractor shall notify the contracting officer, in writing within five (5) calendar days after the occurrence of any of these events. Any replacement of Staffing shall have equal or better qualifications, subject to the approval by the contracting officer.

FACTOR 4: PAST PERFORMANCE

To be evaluated under this factor, Prime Contractor shall provide current and relevant information regarding the Prime Contractor’s actions under previously awarded contracts referenced under Factor 1, “RELEVANT EXPERIENCE OF THE PRIME CONTRACTOR”. If a completed past performance evaluation is available in PPIRS, it shall be submitted with the proposal for each project referenced in Factor 1, “RELEVANT EXPERIENCE OF THE PRIME CONTRACTOR”. If there is not a completed past performance evaluation available in PPIRS, then the Prime Contractor shall submit Past Performance Questionnaires (PPQ) (reference ATTACHMENT 5) for each project referenced in Factor 1, Construction Experience. The Prime Contractor shall notify their client that the PPQ shall be submitted by the reference via email to the Government’s point of contact sean.jackson@va.gov. If the Prime Contractor is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the Prime Contractor shall complete and submit with the proposal the first page of the PPQ, which will provide contract and client information for the respective project(s). The Government may make reasonable attempts to contact the client noted for that project(s) to obtain the PPQ information.

However, Offerors should follow-up with clients/references to help ensure timely submittal of questionnaires.

The Government reserves the right to contact references for verification or additional information.

SPECIAL NOTE 1: For Prime Contractors That May Lack Past Performance Information:

In accordance with FAR Part 15.305(a)(2), in the case of a Prime Contractor that is without a record of relevant past performance, or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance. Such offerors may submit relevant past performance information regarding: predecessor companies;

key personnel who have relevant experience; or subcontractors that will be performing major or critical aspects of this requirement; provided that such information is relevant to the construction in this solicitation.

SPECIAL NOTE 2: For Prime Contractors that have Past Performance: Prime Contractors may provide information on problems encountered on the identified contracts and the offeror’s corrective actions. The government also reserves the right to consider past performance information from all available sources, in addition to any information submitted by offerors.

FACTOR 5: PRIME CONTRACTOR’S SAFETY PLAN

To be evaluated under this factor, the Prime Contractor shall provide a construction safety plan that is deemed acceptable, at a minimum, and meets the following requirements:

a. The contractor's Safety Plan shall be evaluated for their compliance with local/State/Federal rules and regulations and the company's consideration of safety of its employees and the general public. The contractor's Safety Plan at the minimum shall address their safety policy, commitment to safety, detailed disciplinary action to be taken with respect to employees violating safety requirements, personnel safety responsibilities, personnel safety training, personal protective equipment, accident / incident reporting procedure and investigations, emergency procedures guidelines.

b. The contractor’s Safety Plan shall address their plan for completing all contract work on and around an active hospital without impacting patient care and completing all work in accordance with all construction documents and all local/State/Federal rules and regulations.

If the safety plan does not address the requirements listed above (a and b), the proposal will not receive further consideration

FACTOR 6: PRICE PROPOSAL

The Government will evaluate offers in accordance with the process identified in the Price Schedule. The total evaluated price will be determined by the Offeror’s price for all items contained in the price proposal document P09 01 00 00 Appendix A – Bid Form Template or the Bid Items/Contract Line Items determined to be most advantageous to the Government after consideration of funding or other constraints. Offerors must provide a price for each bid item. In addition, offerors shall include a breakdown of material and labor costs, by specification division for each bid item.
