SkyVision Linx 300 Pre-Install • REV4

SECTION 1. INTRODUCTION AND SAFETY

The SKYTRON SkyVision Linx 300 Integration Control System installation can vary according to the specific equipment needs of the OR facility. What is common in all installations is that video and data input sources are routed to various outputs such as displays, video recorders, or networks providing communication and video conferences.

This SkyVision Pre-Installation Manual provides an overview of typical installation considerations and

specificinstallationrequirementsassociatedwithaSkyVisionsystem.

installations with thorough documentation, classroom training, and technical support.

1-1. Safety Precautions The following is a summary CAUTIONS denoted in this manual. These precautions are found throughout the manual where they are applicable. Carefully read the manual before proceeding to operate or service the equipment.

CAUTION

CAUTION without the safety alert symbol, is used to address practices not related to personal injury but with a possibility of damage to equipment.

With Multicast on Parallel, it is very important to properly configure the network segment (VLAN) using multicast to prevent video packets to inadvertently “breach” the “isolated” multicast setup and be transferred over the unicast.

If this occurs, that packet will reach the server and may adversely affect the quality, size and integrity of the video that is being recorded at the same time.

SECTION 2. REQUIREMENTS

2-1. Environmental The SkyVision system must be stored and operated in a clean, dust free environment. Ambient room temperatures should be maintained between 60˚ and 75˚ Fahrenheit with non-condensing humidity between 20% to 50%.

The SkyVision Linx 300 system components will generate up to 2900 BTU's/hr. when fully equipped and under maximum load during peak operating times. Mechanical and HVAC designs of the SKYTRON Nurse Documentation Center (NDC), SORIC system, and Third Party provided versions must address heating, ventilation and exhaust requirements in a manner consistent with the operational requirements of the SkyVision system.

When installing the system within custom cabinetry or a NDC, a minimum of 50 cfm through a 12" square opening is required for constant return HVAC. This will allow the systems to operate within the equipment temperature specifications and ensure product performance, reliability, and longevity.

2-2. Power The SKYTRON Linx 300 System requires two 120VAC/20A, GFCI protected outlets. One circuit is dedicated to the component rack and the second circuit is dedicated to the monitor power supplies.

All receptacle outlet strips, adapters, receptacle covers, and faceplates shall be marked with the following words or equivalent:

"WARNING - TECHNICAL POWER

DO NOT connect to lighting equipment For electronic equipment use only

60/120VAC

GFCI protected" The Uninterruptible Power Supply (UPS) that is provided for power conditioning on the Linx 300 system is not designed for use with Isolated Power Supply systems.

2-3. Grounding SKYTRON requires a minimum 10 AWG copper ground wire to be provided to the 12" x 12" junction box for connection to the equipment rack.

Refer to Article 647.6(B) (Grounding Conductors Required for Sensitive Electronic Equipment) of the NEC.

Permanently wired utilization equipment and receptacles shall be grounded by means of an equipment grounding conductor run with circuit conductors to an equipment grounding bus prominently marked "Technical Equipment Ground" in the originally branch circuit panel board.

2-4. Redundant Connection and Emergency Operation The SKYTRON Linx system MUST BE configured with a redundant back up video. This video delivers a backup video signal to a dedicated display when the system is unable to deliver the primary care video to the display interface. The default primary care video source is dedicated to the endoscopic camera. The customer collaborates with SKYTRON during the installation to identify the source video to connect as the primary and back up video sources.

If the primary video source loses the signal to the display interface(s), and the manual rerouting or use of the preset fails to restore the video image, switch the system to back up video mode. Use the following steps to switch the system to the back up video mode:

1. Manually switch the video mode on the monitor or display 1 to the analog input source.

2. Select the designated back up video input source on the monitor or display 1.

3. If a secondary back up display monitor was designated, manually switch the video source mode on monitor or display 2.

These steps deliver the primary care video source to display on monitor 1 and secondary video on monitor 2.

Contact SKYTRON for assistance to resolve this issue.

2-6. Display Power Management

• The SKYTRON Panasonic and Foreseeson's (FSN) displays can be powered in one of two ways:

• Mount the low voltage power supply onto the display bracket powered by a low leakage 120VAC power line (P/N H5-010-71) run through the boom arm. Mounting the power supply in the back of the display provides open service access to the power supply. Mounting the power supply at this location may not be desirable by the customer for aesthetic reasons, therefore, customer approval is required for this mounting configuration.

• Place the low voltage power supply in a hospital provided NEMA 4 enclosure located in the ceiling or at the equipment rack and run the low voltage power line through the boom arm to the display.

• The Optik View 24" displays come with a power supply and includes 75 feet of cable. The power supply should be located remotely, such as a hospital provided NEMA 4 enclosure (supplied by others) located in the ceiling.

• The remote location of the FSN 24" and 26" display power supply is limited to 75 feet (cable length)

(PN MON-CBL-001).

• The remote location of the Panasonic 26" display power supply is limited to 90 feet (cable length)

(PN DJ-NC0275)

• For integration installations where the rack is within a 75 feet (cable length) of the display, the power supply can be located in the rack. If not, a NEMA 4 encased power supply in the ceiling is recommended (supplied by others).

• The minimum size NEMA 4 enclosure for each power supply is 14”L x 12”W x 7”H (L-Com Global Connectivity – Model: NB1207-10FS-UL or equivalent).

• Appropriate power supplies and cables required for displays not provided by SKYTRON must be

providedbytheequipmentvendortomeetthespecificneedsofthedisplaylocation.
• Notalldisplaysuselowvoltagepowersupplies.Largerdisplays(typically32"andabove)normally

use direct 120VAC input from an outlet as they are intended to be wall mounted. Remote cable kits are not intended for use with these displays as they do not use external low voltage power.

2-7. Optional Equipment A. Image/Video Capture Devices The SkyVision system offers a Capture, Archive, and Streaming system ( SVIS-CAST) for digital capture and streaming, however, some hospitals will maintain existing capture devices and cameras.

When the capture device is located close to the camera, connections can be made by a short copper cable (provided by the Minimally Invasive Surgery (MIS) vendor). Typically, the capture device is located at the nurse’s station far from the camera. The appropriate cable between the camera and the capture device must be provided by the equipment vendor. The SkyVision system will process the capture device output signal, rather than the camera output signal (to maximize functionality), and route the output signal, as limited by the technology of the capture device unless specified otherwise on the OPI document.

B. Optional Remote Trigger Image/Video Capture Capture devices specific to endoscopic cameras allow the surgeon to capture images or video using the trigger on the camera. This method is limited to capturing only the camera source as the surgeon would need to have the camera in hand when capturing. If capturing endoscope images with the endoscope trigger is required, this must be specified by the sales representative as a "special" on the OPI documentation.

The hospital must supply SKYTRON with the make and model number of the camera and capture device so SKYTRON can verify compatibility, proper operation, and provide the required items to quote.

C. Optional Keyboard/Mouse Switch The KM Switch (SVIS-KM) allows a single keyboard and mouse to control up to four (4) PCs or one CAST and three (3) PCs.

When a CAST is purchased in addition to the on-board PC, the KM Switch must be quoted. If the hospital uses their PCs instead of purchasing the SkyVision PCs, they may use their keyboard and mouse instead of the SkyVision keyboard and mouse. The hospital computer can be placed anywhere in the room with their keyboard, mouse, and monitor. If they would like to use the SkyVision keyboard and mouse, a KM Switch is required and the hospital PC must be located within 10 feet of the rack.

D. Additional Touch Panels Optional second and/or third touch panels (SVIS-SECTP) may be selected with a SkyVision system to enable the system to be operated from more than one nurse documentation station location. In this configuration, the second and/or third touch panels are routed through the primary touch panel through VNC connections.

2-8. Remote Rack Locations The stock cable length for a SKYTRON SkyVision System installation is 100 feet. 200 and 300 feet cable lengths may be selected from the OPI document if longer cable lengths are required.

For installations requiring greater distances than 300 feet, consult with the SkyVision Product Manager so the options can be specified and project details/requirements resolved.

2-9. Connections The SkyVision system connects multiple types of video sources (VGA, DVI, SDI, HD-SDI or S-Video/ Composite) to the control rack through conduit runways over single strand multi-mode optical fibers via source conversion faceplates (Figure 5) located near the source. The conversion faceplates are installed in the boom arm equipment carriers or in double-gang electrical wall boxes (Figure 6) depending on the location of the source. The conversion faceplates convert a copper wire signal (digital or analog) to a fiber-optic digital signal. Low voltage power lines are the only other type of connection utilized to and from the SkyVision system. Copper wiring is only employed between components at the rear of the SkyVision control rack. See Figure 7 and for a typical system build drawing set (plan and elevation views) that shows a layout of the affected room(s), equipment, and cable routing.

SECTION 4. CAST NETWORK RESOURCE PLAN

This section describes the storage capabilities and resources of the CAST system, the recommended type of storage sever, and the network resource being used for both real-time broadcasting and off line viewing modes.

4-1. Central Storage System (CSS) The CSS is a server or a cluster of servers performing the following tasks:

• General storage and online video storage

• On-demand broadcasting

• Authentication, encryption, and conditional access

• Mobile interface

4-2. Small Scale CSS - Recommended Servers Recommended small scale servers for pilot and trial options

• 4 CPU cores, 8 threads (statistics)

• 8GB RAM

• Windows 2008 RD or 2012

NOTICE

External storage is provided by the hospital as an external SNAP or NAS.

Both server types are fully redundant.

4-4. Server Virtualization CAST server software components are capable of being virtualized on any standard virtual machine environment.

4-5. Storage Resource Allocation The video assets are saved as a compressed MP2 transport H.264 files with a bit rate of 1.2 Mbps VBR for SD and 4 to 12 to full HD. Following is a table to calculate an average storage resource per broadcast type.

Broadcast type Avg. size per hour Total hours per 1TB SD, PAL, 720x576 250MB 4,000 Hours Full HD, 1080p 3.6GB 300 Hours

It is recommended that the system be initialized with 1TB of initial storage, thus providing 4,000 hours of storage for SD broadcasting and 300 hours for HD broadcasting.

4-6. Network Resource Allocation All CAST videos are multicast on the network with a broadband of 1.2 Mbps VBR for SD or 4 to 12 Mbps for full HD. Network calculation is simply a linear calculation of the number of concurrently used streams.

In a typical hospital environment, a network allocation for 10 concurrent capture devices capturing an SD stream uses an average sum of 12 Mbps.

NOTICE

CAST broadcasts all streams on a multicast basis. Optionally, network segmentation, to gap over VLANs, would benefit large hospital environments.

Whenmulticastisnotenabledinthenetwork,theCASTsystemswitches

4-8. Network Address Range The CAST system uses the following address range and ports (all are configured and can be changed according to local policies);

Protocol Source -> Destination Address range Port Web Services, HTTP Any -> CSS Dynamic 80 Unicast, UDP (when multicast is not available)

Orpheus CAST -> CSS CSS -> Any

Dynamic 5000-5500

Multicast, UDP Orpheus CAST -> Any CSS -> Any Any -> Any (Talkback)

224.10.10.0- 224.255.255.255

4-9. Operating in Unicast In some cases, network segments using CAST devices may not be connected to the server using multicast, for example due to hospital policy or network layout. This section specifies limitations and solutions for devices not connected to the server using multicast.

There are two possible ways to operate in unicast mode:

• ConfiguretheCASTtooperatein"VideoUpload"mode
• MaintaintheconfigurationofCASTtobroadcast,butoverunicast

A. Video Upload Mode When CAST is configured to operate in Video Upload mode, the video is still being recorded on the server while the procedure is being captured. However, CAST uses HTTP to transfer video packets of 2MB to the server. On the server, the file is being created based on these packets.

Depending on network bandwidth allocation and latencies between CAST and the server, these packets may be “backlogged”. If this occurs, CAST stores these packets locally on the device and completes the transfer automatically. Therefore, it may take between 1-5 minutes after the procedure recording has been stopped on the device before the entire video is available on the server. The main difference between this mode and the multicast mode is, in the latter the video is available within a few seconds.

B. Mixed Mode - Multicast on Parallel The hospital network setup may be mixed. This is especially useful if the data center is in a remote location.

For example, if the CAST is deployed in a facility the facility network can be set up as multicast. However between the facility and the data center, the network can be set up as unicast.

In this situation, it is possible to enjoy multicast live streaming between the CAST device and the Show application from anywhere within a facility. However, the video recording to the server will be done through the unicast network (as described in the Video Upload Mode section).

Figure 9. Mixed Mode

CAUTION

With Multicast on Parallel, it is very important to properly configure the network segment (VLAN) using multicast to prevent video packets from inadvertently “breaching” the “isolated” multicast setup and being transferred over the unicast. If this occurs, that packet will reach the server and may adversely affect the quality, size and integrity of the video that is being recorded at the same time.

C. Is Live Stream Possible using Unicast?

Yes, but it’s not recommended due to poor performance issues.

The next diagram shows how the CAST, the Server, and the Show client applications will operate to provide live streaming over unicast. For the example below, a video at 10Mbps bitrate is assumed to be broadcast from the CAST.

1. A single CAST will transmit to the server the video stream. This requires allocating the necessary bandwidth (e.g. 10Mbps) for the stream upload.

2. A single Server will transmit the video stream using the Peer-to-Peer method (like YouTube, Netflix, etc.) to each Show client application. This requires allocating the necessary bandwidth for each Show client (e.g. the Peer) from the server.

As depicted in the diagram below, for 3 Show client applications, the stream will consume 30Mbps (10Mbps x 3) of bandwidth from the Server.

Figure 10. Unicast Livestream To support the peer-to-peer broadcast from the server, the server will require dedicated Network Interface Cards (NICs). The server is commonly deployed as a virtual machine on a VM farm. In order to support a peer-to-peer broadcast in such a deployment, NICs will need to be exclusively allocated to the server, considerably reducing the efficiency of the virtual server farm.

The load on the server will increase considerably, both on disk I/O and CPU to support more than two CAST and/or more than two Show client applications. To support more concurrent streams, more servers will be needed.

4-10. Mobile Server Module – MSM The mobile Server Module is either a SW module in the CSS or a separate module on a separate unit – either a separate machine or on a separate VM. It is important to separate the CSS and the MSM because of a DMZ access to the server – usually mobiles access the system by utilizing the cellular network.

A DMZ could be created by locating the MSM on a separate DMZ or creating a virtual DMZ on a firewall enabling the MSM to connect on a separate NIC in the CSS.

The options are detailed below:

A. MSM on the CSS This option is usually created to serve requests in an internal network (intranet) for the mobile interface.

SECTION 5. CAST STORAGE POLICY PLAN

The CAST system compresses the video signal on the source side and delivers the compressed signal to be stored in real-time. The real-time compression, therefore, points directly to the amount of data to be stored.

5-1. Real-Time Compression The real-time compression is a direct parameter of the source. In some cases it is up to the hospital to define that a specific video source should be compressed only after it has been scaled down:

1. A scaled down source prior of compression could never be reconstructed.

2. No point of scaling down on real-time basis since there is always the option of offline, on a later stage, scale down process.

3. Images taken are not affected by a real-time scaled down compression, images are always kept in the native resolution.

A. Video source type The CAST system can capture the following video source types:

1. Composite PAL/NTSC

2. Analog VESA VGA/XGA/WXGA

3. Digital HD DVI/HDMI/HD-SDI

B. Video Resolution The CAST system can capture the following video resolution types:

1. 720x480/576 P25/30

2. Any VESA 50/60 HZ

3. 720i/P, 1080i/P 30/60

5-2. Offline Compression Offline compression is done after the file had been closed (the video part of the procedure is controlled and stopped using the CAST interface). The compression is done while performing a full transcoding process on the file and it is usually involved in changing or keeping the resolution.

Typical codecs and encapsulations are as the following:

1. Windows7andabove:MP4encapsulatingh.264codec
2. WindowsVistaanddown:WMVencapsulatingWV8

3. Macintosh OSX: MOV encapsulating h.264

4. IOS and Android devices: MP2 encapsulating h.264

5-3. Storage space All compressions mentioned above are on base of VBR (Variable Bit Rate) averaging on a target bitrate within the following table:

Resolution Type Screen Pixel Size Width x Height

Average bitrate Average Size in bytes for Hour of video

SD(StandardDefinition) 720x576 1.2Mbps 500MB
HD(HighDefinition) 1280x720 4Mbsp 1.8GB

Full HD 1920x1080 6Mbsp 2.7GB Full HD 3D 1920x1080 8Mbsp 3.6GB

A. Storage Space Policies The storage space policies are mainly involved in transcoding the original file into other formats and the period to keep them. Following are samples of transcoding to other formats:

1. MOV files for Macintosh users

2. MP4 low resolution to be used by HTML interface

3. MP2 low resolution to be used by smart phone devices

4. MP4 low resolution file to be used a long term storage

5-4. Archival Policies to Define Each department should define archival policies for each procedure. The policies should be defined per hospital server with each department having its own policies. For example, the storage policy for the

surgerydepartmentcouldbedifferentthenthepolicystoragefortheENTdepartment.
Followingarestoragepoliciesthatmustbedefined:

1. Final resolution: should the original be kept or scaled down?

2. Final transcoding: encapsulation and video codec

3. Expiration: how long should the final video be kept?

5-5. Storage Scenarios The following are some hospital storage policy scenarios. In all scenarios an operating room is active on a net base of 4 hours a day 5 days a week (totaling of 1040 hours a year) and is equipped with one
