About this file

This is a Site Visit Form documenting a walkthrough conducted on April 8, 2026, for the Building 4 Elevator Replacement project (Project Number 523A5-18-371) at Brockton VA Healthcare System. The form records contractor compliance with site investigation requirements under FAR clause 52.236-3, establishing that contractors have responsibility for ascertaining work conditions, site accessibility, material handling logistics, utility availability, environmental factors, and ground conditions regardless of items missed during the site visit.

The form details specific areas accessed during the walkthrough, including first, second, and basement floor elevator lobbies; both passenger (P-8) and service (S-9) elevator shafts with tops, cars, and pits; the elevator machine room; rooftop; exterior stairwell; and various electrical and telecommunications rooms. The document notes that contractors declined the Government's offer of ladders for above-ceiling inspections at multiple points during the visit. The form provides a three-phase framework for site visit procedures: pre-visit preparation requiring review of drawings, specifications, and exploratory reports with identification of clarification questions; during-visit documentation of all accessed areas, site conditions affecting transportation and logistics, environmental factors, utility availability, and visual records; and post-visit analysis comparing observations against provided documentation and submitting follow-up questions to the contracting officer. The form emphasizes that contractors cannot use inaccessible areas or discrepancies between site conditions and drawings as justification for additional project costs.

Site Visit

Date: Wednesday, April 8, 2026

Project Name: Building 4 Elevator Replacement (Brockton)

Project Number: 523A5-18-371

Reference FAR clause 52.236-3 Site Investigation and Conditions Affecting the Work

a) The Contractor acknowledges that it has taken steps reasonably necessary to ascertain the nature and location of the work, and that it has investigated and satisfied itself as to the general and local conditions which can affect the work or its cost, including but not limited to (1) conditions bearing upon transportation, disposal, handling, and storage of materials; (2) the availability of labor, water, electric power, and roads; (3) uncertainties of weather, river stages, tides, or similar physical conditions at the site; (4) the conformation and conditions of the ground; and (5) the character of equipment and facilities needed preliminary to and during work performance. The Contractor also acknowledges that it has satisfied itself as to the character, quality, and quantity of surface and subsurface materials or obstacles to be encountered insofar as this information is reasonably ascertainable from an inspection of the site, including all exploratory work done by the Government, as well as from the drawings and specifications made a part of this contract. Any failure of the Contractor to take the actions described and acknowledged in this paragraph will not relieve the Contractor from responsibility for estimating properly the difficulty and cost of successfully performing the work, or for proceeding to successfully perform the work without additional expense to the Government.

(b) The Government assumes no responsibility for any conclusions or interpretations made by the Contractor based on the information made available by the Government. Nor does the Government assume responsibility for any understanding reached or representation made concerning conditions which can affect the work by any of its officers or agents before the execution of this contract, unless that understanding, or representation is expressly stated in this contract.

**Anything additional noted during the site visit that are not shown on drawings or specifications does not absolve the contractor’s responsibility to accurately price the project

Contractors did not have ladders with them for above ceiling inspection. VA offered use of their ladders at multiple points and each time the offer was declined by all present at the walkthrough.

1. Pre-Visit Preparation - Review Contract Requirements

· Study Government-Provided Documentation:

· Drawings and specifications.

· Reports on subsurface or exploratory work.

· Environmental or structural assessments.

· Prepare Questions (if applicable):

· Identify areas in the documentation requiring clarification, such as discrepancies in drawings or specifications.

2. During the Site Visit

· Document all areas visited to formalize what was observed, as per the contract's expectations:

· Clearly note spaces not accessible during the visit and any implications on the scope of work, as noted above.

· Site Conditions

· Transportation and logistics:

· Conditions affecting material transportation, storage, and handling.

· Accessibility of roads and staging areas.

· Environmental factors:

· Weather conditions that could affect work (e.g., flooding risks, high winds).

· Physical site constraints (e.g., ground conformation, river stages).

· Utilities:

· Availability of water, electric power, and other utilities.

· Locations and access points for connection.

· Document above ceiling or subsurface conditions to verify access challenges or hazards.

· Visual Records:

· Photograph areas of interest (permitted areas only).

· Capture potential obstacles or discrepancies in drawings.

· Notes on Observations:

· Record clarifications or additional instructions provided during the visit, submission of RFI’s.

3. Post-Visit Analysis and Reporting

· Summarize Observations:

· Consolidate information about accessed and non-accessed areas, environmental constraints, and site conditions affecting performance.

· Compare Against Documentation:

· Cross-check site conditions with drawings, specifications, and exploratory reports to identify discrepancies.

· Submit Follow-Up Questions:

· Raise concerns about ambiguities or potential risks with the contracting officer.
