13.) Confirm if third-party rescue team must be physically onsite during shaft entry or if standby rescue plan is acceptable.
RESPONSE:
As the Government understands the question, the contactor is asking the following: Is onsite rescue gear, setup to retrieve the entrant, acceptable as an alternative to an on-site rescue team?
The requirement is for a Permit Required Confined Space (PRCS) rescue team to respond in a timely manner (interpreted to be 15 minutes). Ideally, the PRCS rescue team should not need to enter the shaft, and be able to retrieve the entrant from the bottom of the shaft using equipment. However, the things that make the elevator shaft a PRCS are the moving cart, the associated systems (i.e. pneumatic, electrical, and hydraulic) that move the car, and the atmosphere in the shaft itself. If all of those hazards can be mitigated (car and systems locked out/tagged out, or otherwise secured to prevent movement, and atmosphere to be non hazardous and stable), then the shaft can be re-classified as non-PRCS for a set amount of time (typically 8 hour shifts). However, VA Safety and VA IH still reserve the right to require additional measures, such as continuous monitoring to be checked at random times. Additionally, if the general contractor cannot adequately satisfy the requirements to re-classify as non-PRCS, then they will be required to have a third-party rescue team physically on site during shaft entry.
The general contractor is responsible for providing all Safety, OSHA, and VA requirements related to a permit required confined space safety plan, and activity hazard analysis, prior to approval of PRCS entry. This includes, but is not limited to, PRCS and Non-PRCS forms, Air Monitoring, Ventilation, PPE, Emergency Procedures, Training Certifications, Emergency Extraction Plans, Rescue Team Credentials, Plan of Action and request for PRCS Re-Classification with backup documentation, etc. VA Safet, IH, M&O, Facilities, and Projects will review all documentation and an in-person meeting may be required at the site (PRCS) to review documentation.
14.) Confirm background check and badging process duration for all workers.
RESPONSE:
See Specification 01 00 00 GENERAL REQUIRMENTS and para 1.5.B for Security Procedures and information describing the criteria for each type of badge. See also para 1.5.B.2 Contractors are required to submit to the Contracting Officer a list of personnel to be mobilized for the work associated with the contract/task order scope of work to be completed under this specification to the CO and COR. The list is to include current status of SAC background investigation, i.e. – SAC + NACI, date completed; SAC, date completed; pertinent certifications.
15.) Onsite field office trailer location
RESPONSE

Contractor shall coordinate & schedule a meeting/site visit with VA COR, VA M&O Foreman and VA Electrician to review and discuss proposed temporary power routing, building entry, breaker identification and affected areas by tie-in. Contractor shall coordinate with the VA to install necessary breaker in the existing spare for tie-in. This will require OSHA cards and MA electrical licenses for those performing the work and a written request from Contractor to interrupt utilities. Please see the contract documents for the utility outage written request. It shall include affected areas (i.e., what areas will be without power during this outage). It shall also request VA COR for Lock-Out/Tag-Out as this would be required for VA Lock and Tag as electrical work shall be accomplished with all affected circuits or equipment de-energized. The temp power line - If this is to remain above grade, a ramp will be required to be installed the entire run as to allow landscape equipment to pass over or will be required to be installed below grade per NEC. All work shall be in accordance with the latest edition of NFPA101, NEC, and all Federal, State and local codes. Contractor shall seal all exterior penetrations to match existing conditions and make weatherproof and fire proof. Keep the integrity of the structure. Contractors shall be limited to two (2) on-site storage boxes/containers at a time. Storage boxes/containers shall be removed from site upon completion of construction. Contractor shall request placement of all storage boxes/containers from the VA COR identifying on a marked-up site plan (using Google Earth screen shot) the approximate proposed location of the trailer(s) for VA review and approval. Contractor shall identify temporary power for contractor construction trailer.
