About this file
This is an informational presentation document from the Veterans Health Administration (VHA) Office of Integrated Veteran Care, dated February 13, 2026, titled "Care Coordination Overview." The document is not a federal contract solicitation, RFP, RFI, RFQ, or other contracting instrument, but rather an educational overview of the VHA's care coordination program for internal stakeholder awareness.
The presentation defines care coordination as the process of sharing information and building clear care plans for Veterans. It outlines three levels of care coordination services: Basic level (navigation, information, and referrals with minimal contact); Moderate level (frequent follow-up for disease management, prevention, and education); and Complex level (intensive, team-based support for Veterans with multiple or serious needs). The care coordination navigator role includes creating personalized care plans, preventing gaps in care, managing medical and social needs, and reducing unnecessary appointments. The care coordination team provides support in four areas: navigating care, scheduling, communication, and transition. The document addresses referrals to community providers with clear guidance on the reason for referral and requested services; requests for service as official requests for additional clinical care from community providers; prescription handling (routine prescriptions sent to the referring VA medical facility's pharmacy; urgent prescriptions sent to in-network retail or VA pharmacies); and medical emergency procedures requiring Veterans to seek immediate care at the nearest facility without prior VA authorization. The presentation also describes the HealthShare Referral Manager (HSRM) system, which provides clinical summaries of Veterans' medical history, diagnoses, prior medical procedures, and medications. Online provider resources include External Provider Scheduling, Precertification Requirements, Veterans Health Information Exchange, and Clinical Determinations and Indications.
