About this file
This is an Operations and Maintenance Plan Acknowledgment and Deviations Statement form used in a VA lease solicitation.
The form requires an offeror to acknowledge receipt and review of Operations and Maintenance (O&M) requirements specified in Factor 5 of the solicitation and confirm agreement to perform O&M services in accordance with those requirements. The offeror must certify that any O&M obligations not expressly identified as deviations are accepted without exception. The form provides two options: the offeror may indicate no deviations from VA's O&M requirements, or if proposing deviations, must provide attachments for each deviation that include (1) a clear reference to the specific VA requirement being modified, (2) the proposed deviation or alternative approach, and (3) the rationale including any impact on service level, cost, or risk. The authorized official of the offering entity must sign the statement, certifying it is complete and accurate and that no deviations beyond those attached are included in the proposal. The form requires completion of solicitation information fields including RLP number, facility location, and offer due date/time.
