About this file

This document is a Cigna Dental Benefit Summary for the City of Tucson, covering dental insurance for employees and dependents, with a plan renewal date of 07/01/2025. The Dental Preferred Provider Organization (DPPO) plan offers in-network and non-network coverage through the Cigna Advantage Network, with a $2,000 annual benefits maximum. The plan provides comprehensive dental coverage across four service classes: Diagnostic & Preventive (Class I), Basic Restorative (Class II), Major Restorative (Class III), Orthodontia (Class IV), and Implants (Class IX), with varying reimbursement levels ranging from 60% to 100% depending on the service type and network.

The plan features a modest annual deductible of $25 for individuals and $75 for families, with different coinsurance rates for in-network and out-of-network providers. Notable features include the Oral Health Integration Program, which offers enhanced coverage for customers with certain medical conditions, and a Late Entrant Limitation Provision that reduces payment by 50% for some services for members enrolling outside the standard open enrollment period. The plan also includes specific limitations on service frequencies, such as two oral evaluations per year, two routine cleaning sessions, and restrictions on x-rays, fluoride applications, and prosthetic replacements. Certain services are excluded, including cosmetic procedures, athletic mouth guards, and services deemed medical in nature.
