About this file
This is a Utility Relocation Schedule (URS) document prepared by Frontier Communications for Hillsborough County's CR-584 (Waters Avenue) at Florida Mining Blvd. Intersection Improvements project, with plans dated April 5, 2024 and CIP Number 69600318. The document outlines Frontier's facility relocation, adjustment, installation, and protection activities required for the county road construction project. Frontier Communications estimates 60 calendar days for preliminary work and material procurement prior to county project construction, with 16 estimated calendar days for construction items during the county project. The project involves the relocation and removal of existing Frontier facilities including various conduit sizes, fiber optic cables (600-BT, 900-BT, 48-BFO, 144-BFO), handholes, and vaults across multiple stations along the project corridor from Sta. 187+40 to Sta. 192+50. Frontier working days are limited to normal business hours, Monday through Friday, 7:30 a.m. to 5:00 p.m., excluding holidays, with all relocation activities contingent upon receipt of approved permits and the utility work schedule. The road contractor is required to provide 48-hour notice prior to construction commencement and must maintain communication with Frontier's representative, Albert Rivas, throughout the project duration.
The work schedule was signed and approved on October 8, 2025, by Manny Santos as Project Manager and Sandra Gonzalez, P.E., as Section Manager for Hillsborough County, with final approval dated October 9, 2025. Frontier Communications designated Kraivuth Choeykrajang as the UAO Project Representative and Paul Lineberry as the UAO Field Representative, both listed as authorized contacts for project coordination. Key special conditions include Frontier's requirement for a copy of the Critical Path Method (CPM) schedule upon approval by Hillsborough County, prohibition of work during adverse weather conditions per OSHA regulations, and Frontier's right to maintain access throughout the project duration to monitor and maintain facilities for service continuity. The document confirms that no changes were made to the standard form document, and all parties acknowledge their understanding and acceptance of the specified work schedule and coordination requirements.
