Utility Relocation Work Schedule
This document is a Utility Relocation and Adjustment Schedule (URS) prepared by CenturyLink (Level 3) for Hillsborough County, Florida, detailing utility work coordination for the CR 584 (Waters Ave) at Florida Mining Boulevard Intersection Improvements project (CIP Number 69600318). The project involves the relocation and adjustment of existing fiber optic cable (FOC) in HDPE conduits to accommodate roadway construction activities. CenturyLink's work is divided into two phases: Phase 1 requires three calendar days of preliminary work prior to county project construction for vertical adjustment of eight 1.25-inch and twelve 1.5-inch HDPE conduits with FOC to gain clearance for spread footer placement of mast arm structures between stations 189+92 and 190+45; Phase 2 requires one calendar day of work during construction for holding and adjusting two 2-inch HDPE conduits with FOC during drainage structure placement between stations 190+14 and 190+73. The utility work is dependent upon roadway contractor activities and must be coordinated with proper staking and notification procedures. Work must be performed during normal business hours (8:00 a.m. to 5:00 p.m., Monday through Friday), with 48 hours' notice required for scheduled work and 72 hours' advance notice for work outside normal business hours.
The project representative and authorized utility agent is Matt Anthony, reachable at 239-822-4986 during business hours with an emergency contact number of 800-228-8485 for nights, weekends, and holidays. CenturyLink's work will be performed by its National master contractor in close coordination with the roadway contractor. The utility schedule was signed and accepted by Hillsborough County on October 9, 2025, with final approval by Section Manager Sandra Gonzalez, P.E., on the same date. The document specifies that CenturyLink shall obtain a utility permit, comply with Hillsborough County Utility Agreement Management requirements, provide as-built information, and locate facilities within 48 hours of notice to their representative. No changes were made to the standard form document in this execution.
