About this file
This is an Affidavit of Compliance form issued by the Town of Palm Beach Procurement and Contracts Division that applies to the Moving Services contract opportunity. The affidavit requires bidders to attest under penalty of perjury to compliance with Florida foreign entity laws and anti-human trafficking statutes. Entities must confirm they are not owned or controlled by foreign governments of concern, are not organized under foreign laws, do not have principal places of business in foreign countries of concern, and are not foreign principals as defined under Florida Statutes Sections 287.138, 288.007, and 692.201-692.204. Additionally, bidders must affirm that they do not use coercion for labor or services as defined in Florida's human trafficking statute, Section 787.06.
The affidavit must be executed by an authorized representative of the bidding entity and notarized before a Florida notary public. The document requires completion of entity name, signatory title, date, and notarization information, with the notary certifying the identity of the affiant through either physical presence or online notarization. This compliance affidavit is a mandatory requirement for bidders on the Town of Palm Beach's Moving Services solicitation and must be submitted as part of the overall bid response through the Town's Euna Procurement (Bonfire) eProcurement platform.
