About this file
This Triennial Asbestos Re-Inspection Report documents an asbestos survey conducted by MC Consulting Inc. in January 2011 for the Edward Hines Jr. VA Hospital located at 5000 South 5th Avenue in Hines, Illinois. The report provides comprehensive documentation of asbestos-containing materials (ACM) throughout the facility, including detailed methodologies for assessing ACM friability, condition, and potential for disturbance. The document includes extensive abbreviations, glossary of terms, and classifications for evaluating asbestos materials, using a priority action ranking system aligned with the U.S. Environmental Protection Agency's Asbestos Hazard Emergency Response Act (AHERA) regulations.
The report establishes a systematic approach to categorizing asbestos materials, with definitions for ACM friability (ranging from materials that crumble under hand pressure to non-friable materials), condition ratings (good, fair, poor), and potential for disturbance (high, moderate, low). The prioritization framework includes seven action levels, with Level 1 requiring immediate isolation and removal, and Level 7 focusing on preventative maintenance and reducing potential friability during maintenance activities. The document serves as a comprehensive reference for identifying, assessing, and managing asbestos-containing materials within the VA hospital facility.

ABBREVIATIONS
MC Consulting Inc. A - January 2011
ABBREVIATIONS
This section defines the abbreviations used in the spreadsheets that list and assess, room-by-room, the asbestos-containing materials in each re-inspected building. Spreadsheet information includes:
* Type and percent of asbestos found in asbestos-containing materials (ACM)
* Quantity and condition of the ACM
* Description of the ACM
* Potential for ACM disturbance
* Other parameters needed for assessing and prioritizing response actions
Additional terminology used in the spreadsheets for prioritizing response actions is described in the Glossary of Terms.

GLOSSARY OF TERMS
This section defines the terms used in assessing and prioritizing response actions to asbestos-containing materials (ACM) as presented in the spreadsheets. The assessment of an ACM’s potential to release asbestos fibers into air was based on a visual inspection of its physical condition. Pertinent physical characteristics include the ACM’s friability, degree of damage or deterioration, and potential for disturbance. Response actions were prioritized based on a ranking of the ACM’s physical characteristics in accordance with the U.S. Environmental Protection Agency’s Asbestos Hazard Emergency Response Act (AHERA) regulations.
The following sections describe the following:
* ACM Friability
* ACM Condition
* Potential for ACM Disturbance
* Priority Action Ranking
* Nonsuspect Materials
* Not Accessible
ACM Friability
Y: Material that crumbled when hand pressure was applied, ranging from high friability—very soft material that crumbled with light hand pressure—to low friability—material that crumbled after exerting substantial hand pressure
N: Nonfriable material that could not be crumbled, lightly abraded, or powdered by exertion of hand pressure
ACM Condition
Good: No apparent damage to the material
Fair: Material had less than 10 percent overall damage or less than 25 percent localized damage
Poor: Material had 10 percent or more overall damage or 25 percent or more localized damage
Potential for ACM Disturbance
High: 1. Friable ACM is in an area regularly used by building occupants, including maintenance personnel, in the course of their normal activities.
2. There is a reasonable likelihood that the ACM or its covering will become damaged, deteriorated, or delaminated due to factors such as recurrent damage or changes in the building’s use, operation and maintenance (O&M), or occupancy.
3. The ACM is subject to major or continuing disturbance due to factors such as accessibility, vibration, or air erosion.
Moderate: 1.
Low: The potential for contact with the ACM, influence of vibration, and potential for air erosion are assessed to be low.
Priority Action Ranking
1: Isolate the area and restrict access to necessary personnel. Remove or repair the material as soon as possible.
2: Repair or remove the material, or reduce the potential for disturbance as soon as possible. Follow up with an O&M program.
3 or 4: Repair the material and follow up with an O&M program.
5: Follow up with an O&M program, and reduce the material’s potential for disturbance.
6: Follow up with an O&M program, and further reduce the potential for disturbance.
7: Follow up with an O&M program, and take precautions to prevent the material from becoming friable during maintenance activities.
Nonsuspect Material
Nonsuspect material is building material that should be asbestos-free, and is coded NSFM, NSWM, NSCM, and NSINS, as defined in the abbreviations section. Nonsuspect materials were not sampled.
Not Accessible
Not accessible material is building material that cannot be accessed without breaking the integrity of the containing structure, and is denoted as N-ACC in the spreadsheet.
