About this file
This is an attachment to a Request for Proposals (RFP) issued by the New Mexico Department of Health (DOH) for the construction of a new office building in Roswell to serve the Public Health Division. The project scope includes the construction of approximately 9,800 square feet of new single-story, Type IIB fully sprinkled office space and associated site improvements to house PHD offices serving Chaves County. The procurement is governed by the Competitive Sealed Proposals for Construction and Facility Maintenance, Services and Repairs rule per New Mexico Administrative Code (NMAC) 1.4.8 and will result in a single source award to one contractor. The contractual agreement will be exclusively between the State of New Mexico and the selected contractor, with a potential term extending to eight years from the date of the last signature if necessary. The Construction Documents include additive alternates that will only become part of the work if specifically enumerated in the final Agreement, with future funding potentially required to incorporate additional alternatives.
Attachment G specifies that proposers must submit one electronic copy of their firm's written Safety Plan with the proposal package. This requirement is referenced as section 6.i of the RFP and should be submitted as a single page without tabbing of additional sections. No additional pricing terms, set-asides, funding sources, or incumbent information is detailed in this attachment.
