About this file

This is a Vendor Conflict of Interest Disclosure Form required by the City of Tallahassee for the Pump Station 92 Replacement project in Florida. Vendors competing for this infrastructure construction project must disclose any current or reasonably foreseeable conflicts of interest by responding to six specific statements regarding employment relationships with City officials, financial interests held by City employees in the vendor, payments or provisions of value to City personnel, unfair competitive advantages, and conflicts between the vendor's services and affiliated business activities. A mandatory pre-bid meeting is scheduled for March 16, 2026, at 2:00 PM EST via Microsoft Teams. Vendors must answer each statement with "Yes," "No," or "None Known," and any affirmative response requires submission of a detailed explanation as a separate attachment.

The Pump Station 92 Replacement project involves construction of a new quadruplex submersible pump station with above-ground valve array to replace the existing station located at 6636 Buck Lake Road on City-owned property. The comprehensive scope includes deep foundations using micropiles, stormwater retention, sanitary and force main piping, electrical and generator installation, motor control equipment, and complete demolition and decommissioning of the existing facility. The form requires an authorized representative with sufficient knowledge of the vendor's operations to certify in writing that all disclosed information is true and complete. Vendors must provide their name, position title, email address, phone number, and signature with the certification.

Text version

Vendors are responsible for disclosing a current or reasonably foreseeable conflict of interest involving the City of Tallahassee. The City of Tallahassee seeks to avoid, neutralize or mitigate significant potential or actual organizational conflicts of interest. Respond to each of the six statements in the boxes below. Answering “Yes” will not necessarily disqualify a Vendor.

Select “Yes” if a potential conflict of interests exist or if you are uncertain whether a particular circumstance constitutes a potential conflict of interest. If ‘Yes’ is selected, you must provide an explanation on a separate document and attach to this form.

Select “No” if a potential conflict of interest does not exist.

Select “None known” only if (a) it is unduly burdensome to discover the correct response for reasons such as your workforce is so large that it is unreasonable to ascertain whether any potential conflict of interest exists and (b) you in fact do not have knowledge of any potential conflict of interest. If None Known is selected, provide an explanation on a separate document and attach to this form. The explanation must describe why it is unduly burdensome to answer Yes or No.
