SUBJECT: EXCEPTIONAL FAMILY MEMBER PROGRAM
DATE: 08/16/23
REFERENCES: OSD Military OneSource EFMP Training Resource Compendium (Attachment 10)
PURPOSE: To provide direction and guidance on Military OneSource Program and the Exceptional Family Member Program (EFMP). The Contractor is responsible for ensuring Military OneSource staff and network providers adhere to the policies and procedures pertaining to the EFMP.
POLICY: The EFMP may be of significant interest and assistance to active duty and activated Guard and Reservists Service Member (SM) and Family Members (FM). Military OneSource Triage Consultants and Specialty Consultants must be aware of this program; and during the intake assessment must be prepared to educate and connect the SM/FM with the appropriate EFMP resources and contacts.
PROGRAM DESCRIPTION: The EFMP supports military families with special medical and/or educational needs. The program has three components: identification and enrollment (by Medical), assignment coordination (by Medical and Personnel), and family support (by Military Family Support Centers).
EFMP ENROLLMENT IS MANDATORY FOR ALL ACTIVE-DUTY SERVICE MEMBERS who meet enrollment criteria as defined in DoDI 1315.19.
“A service member who knowingly fails or refuses to provide the required information or who knowingly provides false information may be subject to denial of command sponsorship, administrative sanctions, or punishment under either Article 92 (dereliction of duty) or Article 107 (false official statement), Uniform Code of Military Justice.”
Points of contact for EFMP family support are:
Installation EFMP Family Support providers can support families from any branch of Service. Service-specific points of contact for EFMP Family Support are:
· Army – EFMP Manager at Army Community Service
· Marine Corps – EFMP Family Case Worker at Marine Corps Community Services
· Navy – EFMP Liaison at Fleet and Family Support Centers
· Air Force – EFMP Family Support Coordinator at Airman and Family
EFMP IDENTIFICATION AND ENROLLMENT
· Is mandatory for all active-duty Service Members across the Services who meet enrollment criteria as defined in DoDI 1315.19.
· Identifies family members with special medical and/or educational needs.
· Documents the services they require to support their special needs.
EFMP ASSIGNMENT COORDINATION
· Ensures family member’s medical and/or educational special needs are considered in the assignment process.
· Occurs when Personnel command requests a medical and/or educational review of documented needs.
· Determines availability of services at projected location.
EFMP FAMILY SUPPORT: Assists families with special needs by helping them identify and access programs and services. Includes:
· Information and referral for military and community services
· Education and training about issues related to special needs
· Referral to other family center providers
· Local school and early intervention services information
· Warm handoffs to EFMP Family Support at the next location
· Offer family needs assessments and individualized Services Plans
· Non-clinical case management
Family support providers are primarily located at installation Military and Family Readiness Centers. Service members do not need to be enrolled in the EFMP to receive family support services.
PROCEDURE: During any assessment process, the Military OneSource Specialty Consultant should inquire if the SM/FM has a family member with special medical and/or educational needs and if the SM is enrolled in the EFMP. Refer to DoD Instruction 1315.19, “Exceptional Family Member Program”, 23 June 2023 for more information.
If a family member has medical and/or educational needs, and the SM has not already enrolled in the EFMP then the Specialty Consultant should assist the SM in locating the supporting EFMP enrollment point of contact for the Service Branch and completing the EFMP enrollment forms (DD Form 2792 Exceptional Family Member Medical Summary and/or DD Form 2792-1 Exceptional Family Member Special Education/Early Intervention Summary).
Resources can be located by speaking with the EFMP Medical staff (or individual trained for EFMP coordination) at the nearest MTF (for enrollment) or Military and Family Readiness Center for EFMP Family Support services.
Military OneSource special needs consultants will consistently liaison and refer families to the Service Branch assigned installation EFMP Family Support providers during consultations, utilizing 3-way calls whenever possible. Community resources “outside the gate” may need to be further research by consultants depending on the needs or requests of the family.
EXCEPTIONS: There are no exceptions to this policy without Government Military OneSource Program Office written approval.
