About this program
The Department of Health and Human Services Health Resources and Services Administration funds rural healthcare services across multiple program tracks under CFDA 93.912, with the most recent grant award made on September 30, 2026. The program remains actively funded with strong recent award activity—18 awards in 2026 year-to-date, 142 in 2025, and 233 in 2023, indicating sustained federal investment in this space.
The Rural Healthcare Services Programs umbrella encompasses several distinct funding lines: the Rural Maternity and Obstetrics Management Strategies Program establishes collaborative networks to improve access to maternal and obstetric care in rural areas, including services for prenatal, labor, delivery, and postpartum care addressing maternal mortality and morbidity. The Rural Health Care Services Outreach Program supports organizations promoting expanded healthcare delivery to rural underserved populations through consortia-based innovation. The Rural Health Network Development program funds integrated health networks working toward three legislative aims—achieving efficiencies, expanding access and care quality, and strengthening the rural health care system. The Small Health Care Provider Quality Improvement Program builds capacity in small rural facilities for data-driven quality improvement with focus on chronic disease outcomes. The Rural Community Health Support Program provides technical assistance to community-based organizations and rural health stakeholders. Additionally, the Rural Communities Opioid Response Program addresses substance use disorder and opioid use disorder through prevention, treatment, and recovery services, with specific tracks for pathways (workforce development) and overdose response (immediate services).
Assistance provided is predominantly Project Grants (94 percent, 3,668 awards) with a smaller Block Grant component (6 percent, 252 awards). Combined federal dollars obligated total $3.17 billion across 3,921 awards with an additional $60 million in non-federal cost-share, indicating programs with variable match requirements. Two distinct HRSA divisions fund under this listing, jointly accounting for 100 percent of awards: one division provided 2,137 awards totaling $2.5 billion federal dollars obligated, while the second provided 1,784 awards totaling $672.9 million federal dollars obligated.
Recent awards demonstrate the program's concrete scope. Golden Valley Memorial Healthcare received $999,906 for a four-year project establishing maternal and obstetrics care in western Missouri serving 1,400 pregnant and postpartum individuals across six counties with enhanced prenatal care, mental health support, and substance use disorder services. The University of Kentucky College of Social Work received $1,794,316 for the Kentucky Rural Moms Network, a four-year workforce development and care coordination initiative addressing perinatal mental health disparities through provider training, telehealth, and community-based education across rural and Appalachian Kentucky. Rural Wisconsin Health Cooperative received $999,869 to develop creative maternity service delivery models, quality improvement infrastructure, and retention strategies across its network.
Multiple grant opportunities remain open or recently posted. The Rural Health Network Development Planning Program (HRSA-27-048, posted August 13, 2026) supports one-year planning for integrated rural healthcare networks addressing all three legislative aims, with eligible applicants including nonprofits, for-profits, tribal governments, and state/local entities with demonstrated experience serving rural populations. The Rural Maternity and Obstetrics Management Strategies Program (HRSA-27-047, posted August 13, 2026) funds collaborative improvement networks as cooperative agreements for four-year periods to expand maternal and obstetric care access. The Rural Health Network Development Program (HRSA-27-051, posted August 13, 2026) offers two tracts: the Network Advancement Tract supporting existing independent rural healthcare networks to offset structural barriers, and the Network Development Tract supporting nascent networks with planning and operational expansion funding. The Rural Communities Opioid Response Program–Overdose Response (HRSA-25-010, posted July 12, 2024) funds grants addressing immediate and short-term substance use disorder service needs through prevention, treatment, harm reduction, and recovery activities in rural areas.
