About this opportunity
The Health Resources and Services Administration is soliciting applications for the Small Health Care Provider Quality Improvement Program to fund quality improvement activities in small rural healthcare facilities, with applications due August 6, 2026, at 11:59 p.m. ET.
Eligible applicants include rural domestic public or nonprofit private healthcare providers and networks of small rural providers located in HRSA-designated rural areas. Applicants representing the following categories may apply: 501(c)(3) nonprofits, non-501(c)(3) nonprofits, state governments, county governments, city/township governments, tribal governments (federally recognized), tribal organizations (not federally recognized), special district governments, higher education institutions (public), and other entity types. Applicants must not have previously received an award under this program for the same or similar project, though prior recipients may reapply if expanding scope, targeting new populations, or adding network members.
HRSA will award approximately 20 grants in the $0–$250,000 range from a total of $5 million in available Fiscal Year 2026 funding. The four-year performance period runs from September 30, 2026, through September 29, 2030, comprising four 12-month budget periods, with expected award notification by September 1, 2026.
Projects must focus on a clinical health service area such as care coordination, chronic disease management, behavioral health integration, or pulmonary/cardiac rehabilitation. Recipients must implement three core quality improvement strategies: developing and implementing quality improvement strategies with staff training; strengthening data collection through electronic health records and technology tools; and implementing billing and coding strategies to drive revenue and financial sustainability. All recipients receive technical assistance at no cost and must collect and submit annual performance data to HRSA.
Merit review criteria are Need (20 points), Response (25 points), Performance Management (15 points), Impact (15 points), Resources and Capabilities (20 points), and Support Requested (5 points). Applications must not exceed 50 pages and must include required attachments covering rural eligibility, nonprofit status, state office consultation, detailed work plans, organizational charts, staffing plans, and biographical sketches of key personnel. This opportunity is assigned to CFDA 93.912, Rural Healthcare Services Programs.
Synopsis
The purpose of the Small Health Care Provider Quality Improvement Program is to strengthen the quality improvement culture in small rural healthcare facilities by building capacity to effectively collect and use clinical data while implementing evidence-based approaches to improve health care quality with a particular focus on improving chronic disease outcomes. Additionally, the program strengthens the skills and collaboration of the billing and coding staff in coordination with the front-line clinical staff. By creating a sustainable quality improvement capacity in small hospitals and clinics, these facilities will be better positioned to take part in value-based care models while also enhancing health care status for their patients.
