About this opportunity

The Health Resources and Services Administration is soliciting applications for the Rural Health Network Advancement Program to support networks of independent rural hospitals and clinics in integrated network collaboration, with an application deadline of July 24, 2026. The program will award six grants totaling three million dollars, with awards up to five hundred thousand dollars per year across a four-year period of performance from September 30, 2026 through September 29, 2030.

Eligible applicants include domestic public or private entities—nonprofits, for-profits, faith-based organizations, tribes, federally qualified health centers, community health centers, rural health clinics, hospitals, and rural emergency hospitals—that have demonstrated experience serving or capacity to serve medically underserved populations in rural areas. Applicants must represent an established rural health network comprised of at least three separately owned healthcare providers with their own EIN/UEI numbers, with at least 66 percent of network members physically located in HRSA-designated rural areas. Consultation with the State Office of Rural Health is required.

Two funding tracks are available: the Operational Track, which funds projects improving financial viability through shared operational services and integration strategies, and the Clinical Services Track, which funds projects establishing or strengthening clinical service lines and expanding access to care. Merit review assesses applications on need (10 points), response (20 points), performance reporting and evaluation (10 points), impact (10 points), resources and capabilities (40 points), and support requested (10 points). Post-award requirements include annual progress reports, sustainability plans, federal financial reports, performance measure reports, and a long-term plan outlining continuation after federal funding ends. The opportunity is assigned to CFDA 93.912, Rural Healthcare Services Programs.

Synopsis

The Rural Health Network Advancement Program is a pilot initiative to support networks comprised of independent rural hospitals and clinics in integrated network collaboration. It bridges support to offset small scale structural barriers that make it difficult for rural providers to compete in an increasingly consolidated health care system landscape and is designed to help bring economic efficiencies to small independent rural entities by expanding and enhancing their ability to strengthen operations, preserve existing services, and build new lines of care through integrated network collaboration that preserves local autonomy.
