About this opportunity

The Health Resources and Services Administration (HRSA) of the Department of Health and Human Services is offering the Rural Health Care Services Outreach Program, a community-based grant opportunity aimed at expanding and enhancing healthcare delivery in rural communities. The program seeks applicants that can form consortia of at least three healthcare providers to develop innovative, evidence-based initiatives addressing local rural health needs. Projects will be evaluated based on their ability to increase access to new and enhanced services, utilize community engagement, demonstrate health outcomes, and ensure sustainability. Applications are due by January 27, 2025, with awards expected by April 1, 2025 and a project period of May 1, 2025 to April 30, 2029.

The Rural Health Care Services Outreach Program offers two funding tracks - a Regular Track providing up to $250,000 per year for approximately 40 awards, and a Special Track providing up to $300,000 per year for approximately 10 awards focused on addressing the leading causes of avoidable death in rural areas. Total available funding for fiscal year 2025 is $13 million. All applicants must consult with their State Office of Rural Health and ensure at least 66% of consortium members are located in HRSA-designated rural areas.

Synopsis

This community-based grant program aims to support organizations to promote rural health care services outreach by improving and expanding the delivery of health care services to include new and enhanced services in rural areas. To achieve this purpose, the program also aims to strengthen local resources and capacity in rural communities. Through collaborative consortiums that include three or more health care providers, each community can develop innovative approaches to solve their own unique challenges and factors underlying rural health disparities.
