About this opportunity
The Health Resources and Services Administration is soliciting applications for a single Rural Community Health Support Program cooperative agreement offering up to $4,500,000 in FY2026 funding, with applications due by 11:59 p.m. Eastern Time on July 8, 2026.
Eligible applicants are domestic public or private, nonprofit or for-profit entities, including faith-based and community-based organizations and tribal organizations; individuals are not eligible. The program is restricted to a single award.
The cooperative agreement funds development, delivery, and coordination of nationally available technical assistance supporting community-based organizations and rural health stakeholders serving rural populations to improve and expand delivery and access to quality care. The program prioritizes customized, hands-on technical assistance to Federal Office of Rural Health Policy community-based award recipients, particularly the Outreach program, as well as potential applicants.
Applicants must demonstrate prior experience delivering technical assistance to rural communities, established partnerships with rural subject matter experts and national rural organizations, capacity to serve on a national scale, and ability to deliver technical assistance addressing diverse rural health topics including chronic disease management, telehealth, maternal health, and workforce development. Applicants must address non-duplication of federal funding and demonstrate evidence-informed expertise.
The award period runs from September 1, 2026, to August 31, 2029 (three budget periods). Applications are limited to 60 pages and must include detailed project narratives, budgets, staffing plans, partnership agreements, organizational charts, and attestations documenting expertise and nationwide scope.
The opportunity falls under CFDA 93.912, Rural Healthcare Services Programs.
