About this file

This is a Certified Drug-Free Workplace Report Coversheet issued by the State of West Virginia Purchasing Division for a contract with DHF/William R. Sharpe, Jr. Hospital. The document outlines reporting requirements mandated by West Virginia Code 21-1D-7b, whereby contractors must submit certified drug-free workplace reports annually or upon project completion. The contract involves the procurement and complete installation of a conveyer dishwasher and electric blower-dryer equipment. The contract number is ARFQ WSH2400000005.

The coversheet requires vendors to document drug testing compliance across multiple categories, including pre-employment and new hire testing, reasonable suspicion testing, post-accident testing, and random testing results. Vendors must identify the United States Department of Health and Human Services-certified laboratory performing drug tests, document the average number of employees involved in the project, and provide evidence that education and training on drug-free workplace requirements per West Virginia Code 21-1D-5 has been delivered. For contracts exceeding $25,000, reports must be submitted to the West Virginia Purchasing Division at 2019 Washington Street East, Charleston, WV 25305; for contracts of $25,000 or less, submission is directed to the issuing public authority. The coversheet provided appears to be a template form with incomplete vendor contact information and unchecked certification boxes.
